Operator was driving down the road. He went to pass oncoming vehicle on the right and hit a hole and bounced out of his seat. He landed half way back in his seat.
DUE TO RECENT RAINS AND THE EQUIPMENT BEING USED, GROUND WAS WET AND MUDDY. DAMAGE WAS DONE TO RIGHT FOOT. THE PRESSURE FROM THE HOSE CUT HIS BOOT AND THE TEMP FROM THE HOSE CAUSED 3RD DEGREE BURNS TOT HE TOP OF THE FOOT.
DRIVER DROVE OFF SIDE OF LEVEE AND TURNED TRUCK OVER. YOU COULD SEE BY THE PATH OF THE TRUCK, MADE, THAT THE DRIVER DROVE TO THE RIGHT OF THE PATH THEY HAD BEEN DRIVING ALL DAY

PUTTING CABLE ON DRAGLINE AND SETTING WEDGE. TENSION WAS PUT ON CABLE BEFORE WEDGE WAS SET PASSENLY. CABLE HIT HIM IN MOUTH BUSTED HIS LIP AND BROKE TEETH OFF AND KNOCKED SOME OUT.
