RTI Materials · Fall to the walkway or working surface

Employee was clearing material from under a primary crusher. When he stepped backward he steeped on a rock that caused him to lose his balance. He fell with an outstretched right arm. He sprained his right shoulder (right rotator cuff) as a result of the fall.

The middle finger of the employee was torn when it was caught between a punch that the employee was holding and a strip of metal. Finger had stitches and 2 pins placed in the bone to strengthen the bone. Employee is off work for a week.

Employee was cutting a conveyor belt using a utility knife. The activity took place on the ground. EE was kneeling, pulled knife towards himself, knife slipped out of the part of belt being cut and contacted the EE's right knee.

EE was shoveling material. When he raised up and rotated with his back to empty the shovel full of material, he felt a "pop" in his lower back. Poor body mechanics was a contributor to the injury. More ergonomic training would be beneficial. No rules or regs were broken; no PPE issues were involved and there were no equipment issues. No lost time, just minor restrictions.

Conveyor was being held off ground by chain hooked to excavator while conveyor axle was being removed. Load shifted, conveyor rolled back, chain came undone and conveyor fell to ground hitting and pinning employee.
