Drillers helper was lowering a jack leg down a ladder and over extended his back on the lower left side. He did not work out his shift and tried to relax the rest of the work day.

A three man crew were working on a cave in and were traveling to the surface for lunch break when the flat car jack knifed. When the flat car jack knifed it pinched employee foot between the car and trammer.

EE WAS CONDUCTING INSPECTION OF SHAFT. AFTER THEINSPECTION WAS COMPLET THE EE WENT TO THE SHAFT SKIP. HE GOT ON THE BONNET & RANG THE HOIST UP. THE DOOR WAS OPEN ON THE SKIP & CAUGHT IN THE SH AFT. THE IMPACT THREW THE EE OFF & HE TWISTED HIS LEFT ANKLE WHEN HE FELL.
