Employees were hanging a 2" angle iron 107" long above the v-belts on the 7' crusher. They set the angle iron in place for welding and SE's end fell and it pinched EE's hand between the angle iron and the beam, cutting through EE's glove. EE and SE were wearing Radnor A6 cut resistent gloves and the angle iron pierced through the glove between the thumb and forefinger.

Employee was changing screens when a section of screen media slipped from employee's hands and sharp edge struck employee's upper leg, resulting in a laceration.

Employee was organizing parts in a store room when a small hydraulic cylinder rolled onto employee's finger, resulting in a laceration and three small fractures to the rt. index finger

Employee was strapping down a load on a flat bed trailer. While adjusting chains, employee lost balance and fell from the trailer, landing face forward, resulting in contusions and a minor fracture to a sinus bone.

Employee was removing a clevice from a lifting cable after moving a crusher head. When the clevice pin was removed, tension on the cable caused the clevice to spin, striking the employee on the chin. The employee suffered a laceration to chin requiring sutures to close the wound.

Employee was climbing down from a conveyor frame to the catwalk (approx. 3 ft. below) when they lost grip. As they reached out to catch themselves, allegedly they grabbed the handrail, their body continued in downward motion, and caused a strain to the left shoulder. Employee was examined and released from the local ER. Follow up visit the next day at Occupational Clinic (May 14)

One employee was assisting another employee unload steel plates from a freight truck. After sliding the plates off the truck, they attempted to move one of the plates out of the roadway. At this point the employee felt a pain in his lower back. He was taken for medical evaluation and diagnosed with a lower back strain and put on work restrictions.

Employee was adjusting air conditioning vent when his finger accidentally came in contact with fan blade resulting in a laceration that required sutures.

The Greenville County Sheriff's Department responded to a suicide fatality. The victim's body was discovered in a pond on mine property at 4:25 PM. Plant management was notified at that time and an immediate notification call was placed to MSHA at 4:28 PM. The victim was not an employee, contractor, or in any other way associated with mining operations.

Employee was changing rubber screen cloth at the wash plant screen. As one new screen panel was placed on the upper deck it begin to slide into the discharge chute. Employee was standing on the upper deck of the screen and reached for the screen panel as it fell. The weight of the falling panel pulled him into the discharge chute as it fell.

Tech was reinstalling rim base componets. He had his left hand on componets adjusting to position properly..side ring slipped from tire positioning hand and caught finger tips of fingers on left hand between bead seat band and side flange. Amputated pinky finger at 1st knuckle, laceration index finger, loss of finger tips to ring and middle finger.

Employee helping loosen a hydraulic hose on a 988F FEL. When the hydraulic hose fitting broke loose, he struck his right hand against the loader cowling, dislocating his right ring finger. His finger was splinted and he returned to work that afternoon.

A Hammer Operator breaking a boulder in the pit. The boulder was large and his point was approximately 6 feet off the ground. When the hammer point broke through the rock, the base of the hammer hit the edge of the rock, jarring the excavator and the operator. The operator expressed that his neck hurt and was taken to the doctor. He was diagnosed with a strained neck.

Three employees tilted the rock over the conveyor edge where it dropped against frame and caught employees right thumb between the rock and conveyor idler.

Rubber tired loader operator was running the machine with malfunctioning electrical magnetic locks on the hoist and tilt controls. In the manual mode more effort is required and the operator strained his right arm.

The dry grass along the public highway caught fire and spread to the mine property. The fire was reported it to mine management. A dozer cut a fire break through the woods and the company water truck was utilized to prevent the fire from becoming a real hazard. The local fire department arrived and put the final touches on extiguishing the smoldering remains of the fire.

While employee was operating front end loader, he turned on the air conditioning. When the fan came on, particles blew in and into employees eyes. Employee was taken for medical treatment, where he was administered prescription eye drops.

After cutting a piece of steel, the employee picked it up to load it onto a truck. As he picked it up, he rubbed his arm against the edge of the steel plate, causing a 1" long laceration on his forearm. He was given a tetnus shot and treated with derma-bond. He returned to work the same day.

EE WAS REPLACING A 1/2" ROLL PLATE TO COVER THE OIL DRUM IN THE DISTER SCREEN. THE 1/2" ROLL PLATE WEIGHT WAS APPROX. 140 LBS AND HAD BEEN CUT IN HALF FOR BEFORE HANDLING. THE 1/2" ROLL PLATE WAS SETTING ON A SCREEN BRACE INSIDE THE SCREEN WHEN IT FELL MASHING HIS LEFT THUMB BETWEEN THE SCREEN BRACE AND THE ROLL PLATE.

45381NIC WAS INSTALLING TRAVEL CHAIN ON AN 80D.HE HAD A PUNCH IN ONE SIDE OF CHAIN,TRYING TO LINE IT UP.HE STUCK HIS FINGER IN HOLE WHERE PIN GOES JUST TO SEE HOW MUCH FURTHER THE PUNCH NEEDED 03603OME.AS HIS FINGER WAS IN THE PIN HOLE,THE PUNCH SLIPPED OUT,AND TRAVEL CHAIN FLEW APART WHILE FINGER WAS IN IT.

EE WAS TIGHTENING BOLT ON THE BOTTOM OF A BEARING ANOTHER EE WAS TRYING TO HELP BY MOVING THE FLYWHEEL BACK AND FORTH. THE FLYWHEEL ACCIDENTLY HIT THE WRNECH AND THE WRENCH STRUCK VICTIMS ARM AND WRIST AREA.

EMPLOYEE WAS WALKING ON THE CATWALK UP TO THE JAWCRUSHER.CRUSHER WAS JAMMED.SEVERAL EMPLOYEES WERE WORKING TO FREE IT.EMPLOYEE WAS APPROXIMATELY 10FT AWAY.WHEN ROCK BROKE FREE.SEVERAL PIECES C HIPPED OFF APIECE HIT EMPLOYEE ON THE INDEX FINGER ON RIGHT AHND CAUSING A SMALL LACERATION IT REQUIRED STTICHES BECAUSE IT WAS ON THE KNUCKLE.

EMPLOYEE WAS OPERATING THE D5 DOZER ON ROUGH TERRRAIN.HE WAS BACKDRAGGING A SLOPE.DOZER WAS KNOCKED OUT OF REVERSE.OPERATOR WAS WEARING A SEAT BELT BUT WAS BOUNCED AROUND AND HIT HIS ELBOW ON METAL AROUND SEAT.

EMPLOYEE WAS HELPING TO INFLATE A TUBELESS TIRE THAT HAD GONE FLAT. THE TIRES BEAD HAD SEPARATED FROM THE WHEEL RIM. THE INJURED APPLIED A 4" RATCHET TYPE TIE-DOWN STRAP AROUND THE TIRE TO HOL D IT TO THE RIM SO IT COULD TAKE AIR. ONCE INFLATED THE INJURED TRIED TO REMOVE THE STRAP. ON A SECOND ATTEMPT THE STRAP COME LOOSE AND STRUCK THE EMPLOYEE ON THE HAND. EMPLOYEE WAS TAKEN TO

EMPLOYEE HAD JUST CLEARED A JAW CRUSHER HANG-UP AND THEN SIGNALED A LOADED TRUCK TO DUMP ITS LOAD. EMPLOYEE WAS STILL ON THE OPERATORS PLATFORM BEHIND THE OPERATORS SHACK WHEN THE TRUCK DUMPED . A 4" PIECE OF ROCK CVAME OUT OF THE ROCK BOX, PASSED THRU A GUARD RAIL AT A LOW ANGLE AND STRUCK THE EMPLOYEE. THE EMPLOYEE APPLIED FIRST AID AT HOME TO REDUCE SWELLING. ON 1-4-89 HE REPORT

EMPL WAS WELDING ON RADIATOR BRACKET USING ELECTRIC WELDER. AS SPENT ELECTRODE BECOME STUCK TO STEEL. EMPL WORKED ELECTRODE LOOSE FROM STEEL & AS IT CAME LOOSE HIS RIGHT ARM SWUNG TO LEFT LOOSE FROM STEEL & AS IT CAME LOOSE HIS RIGHT ARM SWUNG TO LEFT BRINGING ELECTRODE IN C ONTACT WITH HIS LEFT FOREARM CAUSING SHALLOW PUNCTURE WOUND & BLEEDING FROM SMALL VEIN. EMMPL WAS TAK

EMPLOYEE TIGHTENING JAW CRUSHER WEDGE BOLT USING 1" AIR WRENCH. BOLT TIGHTENED DOWN AND WRENCH SOCKET SLIPPED AND CHIPPED OFF A SLIVER WHICH GOT BEHIND SAFETY GLASSES AND INTO EYE. EMPLOYEE AT TEMPTED REMOVAL BY WASHING OUT HIMSELF AT HOME ON 6-9-88. EMPLOYEE RETURNED TO WORK WHERE REMOVAL WAS ATTEMPTED BY MAGNETIC LOOP. EMPLOYEE WAS SENT TO COMPANY DR. WHO REFERRED HIM TO EYE DR.

EMPLOYEE WAS IN3THE FEEDER BOX TRYING TO FREE UP A LARGE ROCK THAT HAD BECOME JAMMED ON ONE SIDE OF THE BOX. HE WAS WORKING ON THIS ROCK WHEN SOME OTHER ROCKS FELL AND STRUCK THE BAR WHICH KN OCKED HIS HAND 6P AGAINST THE SIDE OF THE FEEDER. THIS CAUSED SOME BRUISING.

EMPLOYEE WAS LI0TING ROCKS OFF OF A CHAIN USED TO LIFT ROCKS OUT OF JAW CRUSHER DUE TO BRIDGING. EMPLOYEE USED ONE HAND TO LIFT ROCK UP AND USED OTHER HAND TO MOVE CHAIN CLEAR OF THE ROCK. HE HAD TO TWIST H3S UPPER BODY TO DO THIS. APPARENTLY, THE TWISTING MOTION PUT OT MUCH STRAIN ON HIS LOWER BACK WHICH RESULTED IN SOME PAIN WHICH INCREASED DURING THE DAY. ON 8/4/87 HE WAS SENT

EMPLOYEE ALLEGES HE WAS STRADDLING THE JAW CRUSHER HELPING TO INSTALL A CHEEK PLATE WHEN HE FELT A BURNING PAIN IN HIS SIDE

INJ WAS CLEARING CHUTE WITH PRYBAR DID NOT OBSERVE CLEARANCE AND HIT FINGER ON THE SIDE OF FEED HOPPER CUT LEFT INDEX FINGER

CONTUSION TO ABDOMEN AND MULTIPLE MINOR CONTUSIONS. JACK BROKE LOOSE FROM 72" CRUSHER CAUSING INJURED TO BE PINNED BETWEEN CHAIN AND CRUSHER JACK.

REMOVE ROCK FROM CONVEYOR BELT WITH BELT MOVING ROCK BOUNCE OFF.
