MINOR DAMAGE TO THE BOOM TRUCK BUT FA TALLY INJURING THE TIRE TECH. THE AREA WAS AN OPEN FIELD AREA, WEATHER WAS CLEAR, AREA WAS DRY, DAYLIGHT HOURS.

Employees were hanging a 2" angle iron 107" long above the v-belts on the 7' crusher. They set the angle iron in place for welding and SE's end fell and it pinched EE's hand between the angle iron and the beam, cutting through EE's glove. EE and SE were wearing Radnor A6 cut resistent gloves and the angle iron pierced through the glove between the thumb and forefinger.

Employee was strapping down a load on a flat bed trailer. While adjusting chains, employee lost balance and fell from the trailer, landing face forward, resulting in contusions and a minor fracture to a sinus bone.

Employee was removing a clevice from a lifting cable after moving a crusher head. When the clevice pin was removed, tension on the cable caused the clevice to spin, striking the employee on the chin. The employee suffered a laceration to chin requiring sutures to close the wound.

Employee was climbing down from a conveyor frame to the catwalk (approx. 3 ft. below) when they lost grip. As they reached out to catch themselves, allegedly they grabbed the handrail, their body continued in downward motion, and caused a strain to the left shoulder. Employee was examined and released from the local ER. Follow up visit the next day at Occupational Clinic (May 14)

One employee was assisting another employee unload steel plates from a freight truck. After sliding the plates off the truck, they attempted to move one of the plates out of the roadway. At this point the employee felt a pain in his lower back. He was taken for medical evaluation and diagnosed with a lower back strain and put on work restrictions.

The Greenville County Sheriff's Department responded to a suicide fatality. The victim's body was discovered in a pond on mine property at 4:25 PM. Plant management was notified at that time and an immediate notification call was placed to MSHA at 4:28 PM. The victim was not an employee, contractor, or in any other way associated with mining operations.

Employee was changing rubber screen cloth at the wash plant screen. As one new screen panel was placed on the upper deck it begin to slide into the discharge chute. Employee was standing on the upper deck of the screen and reached for the screen panel as it fell. The weight of the falling panel pulled him into the discharge chute as it fell.

He had his left hand on componets adjusting to position properly..side ring slipped from tire positioning hand and caught finger tips of fingers on left hand between bead seat band and side flange. Amputated pinky finger at 1st knuckle, laceration index finger, loss of finger tips to ring and middle finger.

Employee helping loosen a hydraulic hose on a 988F FEL. When the hydraulic hose fitting broke loose, he struck his right hand against the loader cowling, dislocating his right ring finger. His finger was splinted and he returned to work that afternoon.

A Hammer Operator breaking a boulder in the pit. The boulder was large and his point was approximately 6 feet off the ground. When the hammer point broke through the rock, the base of the hammer hit the edge of the rock, jarring the excavator and the operator. The operator expressed that his neck hurt and was taken to the doctor. He was diagnosed with a strained neck.

The dry grass along the public highway caught fire and spread to the mine property. The fire was reported it to mine management. A dozer cut a fire break through the woods and the company water truck was utilized to prevent the fire from becoming a real hazard. The local fire department arrived and put the final touches on extiguishing the smoldering remains of the fire.

After cutting a piece of steel, the employee picked it up to load it onto a truck. As he picked it up, he rubbed his arm against the edge of the steel plate, causing a 1" long laceration on his forearm. He was given a tetnus shot and treated with derma-bond. He returned to work the same day.

EE WAS REPLACING A 1/2" ROLL PLATE TO COVER THE OIL DRUM IN THE DISTER SCREEN. THE 1/2" ROLL PLATE WEIGHT WAS APPROX. 140 LBS AND HAD BEEN CUT IN HALF FOR BEFORE HANDLING. THE 1/2" ROLL PLATE WAS SETTING ON A SCREEN BRACE INSIDE THE SCREEN WHEN IT FELL MASHING HIS LEFT THUMB BETWEEN THE SCREEN BRACE AND THE ROLL PLATE.
