INJURED STEPPED ON THE BACK OF THE PLATFORM TO COUNTER BAL ANCE IT AND WHEN THE FORKLIFT PICKED IT UP,INJURED WAS KNOCKED OFF OF IT STRIKING AXLE OF FORKLIFT CAUSING HEAD INJURY.
