EMPLOYEE (LOADER OPERATOR) WAS FOUND SITTING IN A CHAIR IN THE SCALE HOUSE @ 8:15 A.M. SUFFERING FROM AN APPARENT HEART ATTACK. HE WAS THEN TRANSPORTED TO DUBOIS REGIONAL MEDICAL CENTER. AND AUTOPSY IS BEING PERFORMED. (AUTOPSY REPORT- HEART ATTACK).

LARGE LUMPS OF COAL WERE BRIDGED ABOVE CRUSHER ROLLS IN CHUTE. OPERATOR TRYED TO BREAK THEM WITH TAMPING BAR. BAR WENT BETWEEN LUMPS, STRUCK CRUSHER ROLL, AND KICKED BACK SRIKING HIM ON SIDE OF HEAD NEAR LEFT EYE. HE RECEIVED A 1/2" CUT CORNER OF EYE AND BRUISE.
