BACK TWO DISC IN BACK SLIPPED OUT OF PLACE. JUMPED OFF OF DRAGLINE. WE WERE HITTING BOOM OF DRAGLINE BACK DOWN FROM AN UP ANGLE WHEN LEG OR BOOM BENT. INJURED JUMPED OFF OF DRAGLINE OUT OF WAY BOOM DID NOT FALL.

EE WAS STEPPING DOWN AFTER CLEANING THE CRUSHER HOPPER. HE LANDED ON A 3 INCH ROCK WITH HIS RIGHT FOOT. HIS FOOT SLIPPED OFF THE ROCK RESULTING IN TWISTED ANKLE.

THE INJURED AND ANOTHER EMPLOYEE WERE WORKING TO FREE A CONVEYOR BELT FROM SOME GRATING. NO MACHINERY WAS IN OPERATION AT THE TIME. THE PRY BAR THEY WERE USING SLIPPED, MASHING HIS FINGER.
