Vulcan Construction Materials, LLC · Struck by falling object

The injured employee was working in the North East corner of the pit loading shot rock. While waiting for the haul truck to return to the muck pile, the excavator was struck by several large rocks. No one witnessed the accident. The haul truck driver discovered the accident scene and rescue/recovery efforts begun. The excavator operator was fatally injured.

Employee was pulling gate closed when employee sprained foot. Date of injury was on 4/10/2021, but the incident did not become reportable until 7/7/2021 when employee was prescribed physical therapy.

On 3/12/12 an injury was org. reported #1538802 where a mechanic was inflating a tire on a 988 Cat Wheel loader when the tire failed causing sudden release of air that had been under pressure. During annual audio. it was determined that the EE had a reportable shift in his hearing due to the noise generated from the previous tire failure that had been reported to MSHA.

THE EMPLOYEE WAS STANDING 5 OR 6' FROM THE TRUCK THAT A TIRE WAS BEING AIRED UP. THE TIRE EXPLODED AND AIR PRESSURE KNOCKED HIS GLASSES AND HARD HAT OFF. THE AIR BLISTERED HIS FACE AND EYES AND HE ALSO GOT METAL FRAGMENTS IN HIS EYES.

EE (ASSISTED BY ANOTHER EE) WAS REPAIRING A SECTION OF HANDRIAL BENT BY STONE LEADING INTO PRIMARY CRUSHER. WHILE PULLING ON HANDRAIL TO GET IT ORIGINAL POSITION, IT BROKE AND HE LOST HIS FOOT ING AND FELL TO GROUND.
