The employee was operating CAT loader 980M, feeding the crushing plant. On frequent occasions, EE would hit boulders that were not visible. During the shift, the employee stated that their back was sore.

The employee walked to the back of screen 1, middle section, to shovel material off the deck. As EE walked to the middle section, EE's left shoulder made contact with the screen while it was operating.

The employee was inspecting a leaky radiator on a portable screen. The employee was sliding a steel plate off the engine compartment of a portable screen plant when the weight of the plate transferred down. The hand was on the plate that was lifted up. The finger got caught between the head pulley scraper and the metal plate.
