The employee was installing a hoist cylinder to the dump body of a 960E haul truck. He loosened the bolt on the shipping strap and the bolt threads failed causing the hoist cylinder to fall pinning his left shoulder between the hoist cylinder and the tire which resulted in shoulder trauma.
Shop clean up. Mechanic was moving items out of a wooden box and caught his hand/wrist on a metal shim as he was removing the item from the box which resulted in a lacaration requiring stitches.

Employee strained lower back while moving wood.
While working on a 960 E haul truck the employee fell off a ladder and cut the back of his head on a bracket. This resulted in two stitches. Employee was released and returned to work the same day.

THE EMPLOYEE HAS BEEN WORKING IN THE DRILLING AND BLASTING AREA FOR 12 YEARS ON UNEVEN GROUND. ON 5/25 HE WAS ON THE GROUND WORKING BY A DRILL. HE JUMPED UP SLIGHTLY TO RETRIEVE SOME MATERIAL OFF THE DRILL DECK. AS HE LANDED HE EXPERIENCED PAIN IN HIS RIGHT KNEE. HE HAD SIMILAR INJURIES ON TWO OCCASSIONS AWAY FROM WORK. HE HAS CONTINUED TO WORK BUT WILL EVENTUALLY NEED SURGERY.
Ee was using a ladder to access the top of a haul truck frame. On his first attempt to climb the ladder and tie it off, the bottom of the ladder kicked out. The ee fell approx five feet, landing on the ladder as it struck the shop floor. He received a cut about 1 1/2" long just below his left knee cap.
Employee was removing a motor from dragline. Another ee was using hoist to lift the motor. EE reached into the area to remove shims. When he did this, hoist operator moved the load resulting in EE getting his finger caught between the side of the foot and steel plate.
Placed R. foot on sill step of locomotive, grabbed onto hand-holds and started to climb ladder, felt discomfort in foot.
The ee was operating a cat truck excavator doing clean-up work in the coal pit. When a soocer ball sized rock came off the highwall and broke through the cab window. Broken glass or a fragment from the broken rock caused a 2" laceration on the right side of the ee's head. He was transported to the hospital ER where 7 staples were required to close the wound.

THE EE WAS DOING REPAIR WORK ON RUBBER CURTAINS AT THE COAL TRUCK DUMP. AS HE HELD THE MATERIAL TO STEADY IT WITH HIS LEFT HAND, HE BEGAN TO DRILL INTO RUBBER BELTING. AS THE DRILL BIT PENETRA TED THE RUBBER, IT CONTACTED THE INDEX FINGER ON THE LEFT HAND. SIXTEEN STITCHES WERE REQUIRED TO CLOSE THE WOUND.
MINE WORKER DROPPED ALUMINUM CHANNEL ON EMPLOYEE'S HEAD.
SLIPPED ON ICE WHILE BENT OVER AND DISLOCATED KNEE.
SOMETIME DURING THE SHIFT THE EMPLOYEE GOT A PIECE OF METAL IN HIS EYE. HE DOESN'T KNOT HOW. WHEN HE GOT HOME HIS EYE STARTED TO HURT AND HE WENT TO THE EMERGENY ROOM WHERE A PIECE OF METAL WA S REMOVED FROM HIS EYE.
WHILE CHECKING FOR AN AIR LEAK IN THE AIR FILTER CABINET, A FITTING/LINE BROKE. THE EE WAS POSITIONED IN FRONT OF THIS LINE. THE AIR BLAST PUSHED THE EE'S HEAD BACK AGAINST METAL STRUCTURE WIT HIN THE AIR BOX. THE RIGHT SIDE OF THE EE'S EAR WAS CUT REQUIRING STITCHES TO CLOSE THE WOUND. DUST WAS ALSO BLOWN INTO THE LEFT EYE, WHICH WAS FLUSHED OUT.
THE EE WAS ATTEMPTING TO GREASE A DOZER BLADE LIFT CYLINDER WHEN GREASE FROM THE HIGH PRESSURE LUBE SYSTEM GLANCED OFF THE CYLINDER AND PENETRATED THE UNDERSIDE OF THE EE RIGHT FOREARM. A SMALL INCISION WAS REQUIRED TO REMOVE A PEA SIZEGLOB OF GREASE. THE WOUND REQUIRED TWO STITCHES.
WHILE HAULING OVERBURDEN FROM A 2800 P & H ELECTRIC SHOVEL, A LIEBHERR 240 TON HAUL TRUCK CAUGHT ON FIRE. IT HAD REACHED THE DUMP SITE AND JUST PRIOR TO HOISTING THE BOX THE OPERATOR SAW FLAME S ERUPT FROM UNDER THE CAB AND FRONT DESK AREA. HE SHUT THE UNIT DOWN, INITIATED THE FIRE SUPPRESSION SYSTEM AND EXITED THE TRUCK. MINE RESCUE UNITS & COUNTY FIRE EQUIP EXTINGUISHED THE FIRE
WHILE STANDING ON THE RIGHT FRONT TIRE OF THE REAR TANDEN ON A MOTOR GRADER, THE EE HAD JUST FINISHED CLEANING THE INSIDE OF DOOR GLASS, WITH THE DOOR SWUNG OPEN. AS HE STEPPED BETWEEN THE TIR E AND THE CAB, TO ENTER THE CAB, THE DOOR WHICH HE WAS HOLDING ONTO SWUNG FREE (THE HOLD OPEN LATCH ON THE DOOR WAS NOT LOCKED IN). THE EE FELL TO THE GROUND LANDING ON HIS LEFT HIP, FRACTURI
THE EE WAS LOOKING FOR THE SOURCE OF SMOKE AFTER WELDING OPERATIONS ON A 2800 SHOVEL. HE FELT HEAT ON A MANDOOR AND WHEN HE OPENED THE DOOR FLAMES AND SHOT OUT AND BURNT HIS ARMS AND FACE.
A SEVERE THUNDERSTORM DROPPED 2.04 INCHES OF RAIN OVER A 2 HOUR PERIOD. WATER RUNOFF BUILT UP ON A SPOIL DUMP. IT EVENTUALLY BREACHED THE DUMP BERM, AND FLOWED DOWN INTO THE COAL PIT ACCESS RA MP. THE 24K MOTORGRADER HAD ENTERED THE PIT (WITH 2' OF WATER IN THE BOTTOM) TO PICK UP THE COAL SHOVEL OPERATOR. AS THEY WERE EXITING THE PIT, WATER ENGULFED THE GRADER. TWO EE'S WERE RESCUE
THE EE WAS WORKING INSIDE THE SHOP FACILITY AT A WELDING TABLE. HE WAS ATTEMPTING TO INSTALL A BEARING IN A PART BORE. AS HE DROVE THE BEARING IN, A PIECE OF THE BEARING SPLINTERED OFF AND WAS PROPELLED INTO THE MUSCLE AREA BELOW THE THUMB OF EE'S LEFT HAND. A DOCTOR'S VISIT WAS REQUIREDTO REMOVE THE OBJECT.
TIRE HAD BEEN MOUNTED, WAS ON THE TRUCK, BEING AIRED UP, BUT WOULD NOT SEAT. EE WAS REACHING INBETWEEN RIGHT REAR DUAL APPLYING LUBRICANT AROUND TIRE WITH FINGERS. TIRE SPRANG INTO POSITION A ND BEADSEAT BAND SNAPPED AGAINST LOCK RING. THISMOVEMENT CUT FINGERS AT THE PINCH POINT. LEFT HAND 3RD & 4TH FINGER ENDS AMPUTATED.
WHILE HELPING AN ELECTRICIAN MOVE A BLOWER FAN MOTOR, THE EE'S LITTLE FINGER ON HIS LEFT HAND WAS PINCHED AND CUT. FOUR DAYS LATER INFECTION DEVELOPED. THE EE WAS ON SCHEDULED DAYS OFF. HE REC EIVED TREATMENT AT THE CCMH EMERGENCY ROOM CONSISTING OF IV ANTIBIOTICS AND A TETANUS SHOT.

EE WAS ASSISTING MECHANIC HELPING TO REMOVE A SIDE FRAME PIN FROM THE EQUALIZER BAR ON A475 KNOMATSA DOZER. HE WAS HOLDING A BAR AS THE PIN WAS DRIVEN OUT. WHEN THE PIN CLEARED THE BAR SLID IN TO THE BORE OF THE EQUALIZER BAR, WITH THE FRAMEWEIGHT OFF THE EQUALIZER, IT ROTATED UP, TWISTING THE BAR AND PINCHING LEFT THUMB.
EE WAS OPERATING THE LUBE/FUEL TRUCK, AND IN THE PROCESS OF SERVICING A KOMATSU 475 DOZER. HE WAS ON THE CAB DECK LEVEL AND ATTEMPTING TO DISMOUNT TO TURN ON A PUMP WHEN HE SLIPPED FROM THE DE CK OR TRACK AREA, AND FELL TO THE GROUND. THE FALL CAUSED A LATERIAL TIBIAL PLATEAU FRACTURE TO THE RIGHT LEG.
EE WAS RIDING IN THE BUDDY SEAT OF UNIT #2409 TRAINING A NEW EE. THE TRUCK HIT 3 BUMPS CAUSING THE SUSPENSION TO BOTTOM OUT. THIS INCIDENT, COMBINED WITH OTHER NON WORK RELATED BACK PROBLEMS CAUSED DISCOMFORT WHICH WAS INITIALLY DIAGNOSED AS A LOWER BACK STRAIN. ADDITIONAL MEDICAL TREATMENT REVIEWED DAMAGE TO LOWER BACK DISCS.
WHILE MOVING DRAGLINE TRAIL CABLE WITH A PICKUP, A LENGTH OF CABLE WAS BEING PULLED UP. THERE WAS A LOOP IN THIS LENGTH OF CABLE AND AS IT WAS PULLED STRAIGHT, THE CABLE STRUCK EE IN THE LEF T ANKLE.
COWORKER WAS LOERING AN HYDRAULIC PUMP PART INTO POSITION, WHILE EE WAS ATTEMPTING TO LINE THE PART UP WITH A RIGID IN PLACE HYDRAULIC LINE. DURING FINAL ALIGNMENT EE'S INDEX FINGER WAS CUT.
WHILE MOVING FROM CAB TO FRONT OF DOZER TRACKS TO DISMOUNT, THE OPERATOR EITHER LOST HIS GRIP ON THE HAND GRAB RAILS OR SLIPPED ON THE TRACKS CAUSING HIM TO FALL TO THE GROUND. HE SUSTAINED A DISLOCATION & FRACTURE IN THE LEFT ANKLE JOINT.
WHILE ASSISTING MECHANIC, WHO WAS REMOVING HYDRAULIC HOSE WHICH RAN TO TRANSMISSION ON 3312 WATER TRUCK, THE WATER TRUCK OPERATOR FELT PAIN AS HE PULLED ON RIGHT SHOULDER ON THE TRANSMISSION HOSE.
WHILE PULLING TRACK TOGETHER TO PUT IN PIN, COWORKER USED BAR TO HELP BRING TRACK LINKS TOGETHER WHILE EE WAS USING CHAIN COMEALONG, BAR SLIPED AND PUSHED EE'S FACE INTO TRACK PAD CAUSING CUT LIP.

EE WAS INSPECTING BRAKES ON FRONT END LOADER. THE LOADER WAS IDLING AND BLOWER MOTOR OR WIND BLEW FOREIGN OBJECT INTO EE'S L. EYE. EE HAD OBJECT REMOVED AT HOSPITAL ER. EE STATED THAT HE WAS WEARING PROTECTIVE EYEWEAR AT THE TIME.
WHILE OPERATING A HAND HELD METAL GRINDER DURINGREPAIR WORK ON A SHOVEL BUCKET, WIND BLEW GRINDING DEBRIS INTO EE'S RIGHT EYE. EMPLOYEE WAS TRANSPORTED TO CAMPBELL COUNTY MEMORIAL HOSPITAL E.R FOR REMOVAL OF OBJECT AND FURTHER EXAMINATION. WAS RELEASED WITH NO RESTRICTIONS. HE WAS WEARING FACE SHIELD AND SAFETY GLASSES AT TIME OF INCIDENT.
WHILE CHECKING BATTERIES ON HAUL TRUCK 3205 AS PART OF PREVENTIVE MAINTENANCE SCHEDULE, EE WAS LIFTING COVER OFF BATTERY BOX. COVER SLIPPED FROM HIS GRASP AND CONTACTED BATTERY TERMINALS. THE BATTERY EXPLODED AND A PIECE OF THE BATTERY STRUCK THE EE IN THE UPPER ABDOMEN JUST BELOW THE STERNUM.
WHILE EE WAS SERVICING A WATER TRUCK, A FOREIGN OBJECT BLEW INTO AND BECAME LODGED IN HIS RIGHT EYE. THE OBJECT WAS REMOVED BY A PHYSICIAN AND ANTIBIOTIC EYE DROPS WERE PRESCRIBED.
WHILE MOVING HYDRAULIC JACK TO POSITION TO LIFT ENGINE ON 285 DEMAG HYDRAULIC SHOVEL, MECHANIC WAS STRUCK ON THE BACK OF THE HEAD WHEN THE JACKSLIPPED AND FELL. THE IMPACT KNOCKED THE MECH- ANIC OFF OF THE LADDER HE WAS STANDING SEVERAL STEPS UP ON.
THE EMPLOYEE STRAINED HIS BACK WHILE MANUALLY LIFTING A BULLDOZER TRUNION CAP FROM THE BULLDOZER PUSH ARM.
THE EMPLOYEE AND A CO-WORKER WERE ATTEMPTING TO DRIVE A BUCKET TOOTH OUT IN ORDER TO CHANGE A TOOTH. THE EMPLOYEE HELD THE PIN DRIVER AS HIS CO-WORKER USED A SLEDGE HAMMER ATTEMPTING TO HIT THE PIN DRIVER.THE CO-WORKER MISSED THE DRIVER STRIKING THE BUCKET TOOTH CAUSING THE EMPLOYEE TO BE STRUCK BY A METAL FRAGMENT FROM THE TOOTH.
WHILE CLEANING THE BACK CAB WINDOW ON THE WABCO WATER TRUCK, THE OPERATOR INADVERTENTLY STEPPED OFF THE CATWALK AND FELL AGAINST THE HYDRAULIC TANK.
AFTER MOUNTING THE SHOVEL, THE OPERATOR PULLED THE LADDER UP, STRAINING HIS BACK.
UPON ARRIVING TO WORK WITH OTHER ADMIN PERSONNELIN A DODGE VAN, THE EMPLOYEE WHILE EXITING FROM THE SIDE DOOR STEPPED PARTIALLY ONTO THE SIDEWALK CURB. THE EMPLOYEE FELL BACKWARDS STRIKING THE SIDEWALK, LANDING ON THE BACKSIDE.
WHILE HAULING OVERBURDEN MATERIAL, A FIRE BROKE OUT ON 240 TON HAUL TRUCK. AFTER THE TRUCK STOPPED, THE OPERATOR ENGAGED THE DUMP BRAKE, DISCHARGED THE FIRE SUPPRESSION SYSTEM AND DISMOUNTED T RUCK. THE OPERATOR RETIRED TO A SAFE LOCATION AND WAS UNINJURED. THE HAUL TRUCK IS A TOTAL LOSS. MSHA WAS NOTIFIED OF THE INCIDENT.
EE CLIMBED ON VERY SMALL STEP LADDER TO CHECK LEVEL OF HYDRAULIC OIL ON HAUL TRUCK. HE FELL FROM LADDER LANDING ON SHOULDER. HE DIDN'T THINK IT WAS A REPORTABLE ACCIDENT, BUT HIS SHOULDER CONT INUED TO HURT AND HE SOUGHT MEDICAL TREATMENT.
EE WAS PUTTING SHEETS OF STEEL IN A RACK (PRYINGTWO 4X8 SHEETS APART). A SHEET OF METAL FELL TOWARDS HIM, SMASHING HIS FOOT & BREAKING TWO TOES.
WHILE STEPPING OVER AN ELECTRICAL TROUGH TO RETRIEVE A WRENCH UNDER THE #4 DRAGLINE SWING CASE. THE EE SLIPPED CATCHING HIMSELF. WHILE OFF BALANCE THE EE FELT A POP IN HIS RIGHT KNEE.
EE WAS USING A HIGH PRESSURE HOSE TO CLEAN OFF AN AREA ON 2001 TRUCK. HE WAS STANDING ON A LADDER THAT WAS NOT THE PROPER TOOL FOR THIS JOB. WHEN HE LEFT OFF THE PRESSURE ON THE HOSE, HE LOST HIS BALANCE AND FELL TO THE GROUND. HE LANDED ON HIS RIGHT SIDE. THE FALL RESULTED IN AN INJUURY WITH TWO BROKEN RIBS.

THE EMPLOYEE WAS REMOVING THE CUTTING EDGES FROMA DOZER. THE IMPACT WRENCH SLIPPED OFF THE NUT &TWISTED, FORCING HIS LEFT THUMB INBETWEEN THE DOZER BLADE & WRENCH, SMASHING IT.
EE WAS USING A 1/2" AIR IMPACT WITH A 1/2" HEX HEAD SOCKET TO REMOVE ALLEN HEAD BOLTS. WHILE DOING SO THE ROLL PIN WHICH RETAINS THE HEX TO THESOCKET WORKED OUT. USING HIS LEFT HAND TO STABIL IZE & GUIDE THE SOCKET & BOLT, HE RECEIVED A LACERATION TO HIS LEFT PALM WHEN THE ROLL PIN WORKED LOOSE.
THE BULLDOZER BECAME STUCK ON A DOWNHILL SLOPE OF A KEY CUT. THE OPERATOR DISMOUNTED ON THE RIGHT REAR OF THE DOZER. UPON TOUCHING THE GROUND HETURNED AND SLIPPED ON THE LOOSE DIRT FALLING AGA INST THE RIGHT SIDE TRACK CAUSING AN ABRASION & CONTUSION TO HIS RIGHT THIGH.
AFTER TAKING LUNCH BREAK, EMPLOYEE WAS PREPARINGTO PICK UP A 240 HAULTRUCK WHEEL CHOCK PRIOR TO STARTING BACK TO OPERATION. WHILE ATTEMPTING TO LIFT THE CHOCK, SHE FELT PAIN IN HER BACK.
WHILE LOADING COAL TRUCKS WITH LETOURNEAUX WHEELLOADER THE OPERATOR OF THE LOADER REPORTED THAT WHILE TURNING THE STEERING WHEEL HER LEFT SHOULDER POPPED
EMPLOYEE HAD BEEN WORKING IN THE MEZZANINE AREA OF THE WAREHOUSE WHEN THE TELEPHONE BEGAN TO RING. WHILE WALKING DOWN THE STEPS, THE EMPLOYEE INDICATED DISCOMFORT TO THE RIGHT KNEE. THE EMPL OYEE REPORTED THE SAME ON 7/24/00.
WHILE REMOVING BUSHINGS FROM A DOUBLE WALL PARTSFEELER, THE LID SLAMMED CLOSED LACERATING THE EMPLOYEE'S RIGHT THUMB, REQUIRING STITCHES.
TWO EE S WERE PHYSICIALLY MOVING A BAR GRATING CATWALK WEIGHING APPROX. 200 LBX ONE CORNOR EDGE SLIPPED, BOTH EE DECIDED TO DROP THE CATWALK. AS THE CATWALK FELL, ONE EE CONTINUED TO HOLD ON. THE FALLING FORCE CAUSED AN OPEN FRACTURE TO THEEE'S FIRST DIGIT OF HIS LEFT RING FINGER.
WHILE USING A 1 1/2" AIR IMPACT TO TIGHTEN T BLOCK BOLTS ON TO THE HOIST DRUM, THE BOLT SHOULDER AND HEAD CAUSED CUTTING OF THE MILD STEEL. WASHER. PARTICLE OF RUST FROM THE WASHER BECAME AIRB ORNE AND ENTERED THE EE'S EYE BEHIND HIS SAFETY GLASSESS.
TWO WELDERS WERE ATTMEPTING TO REMOVE A TOOTH ADAPTER ON A P&H 2800 BUCKET. AFTER REMOVING EMBEDDED DIRT, OEN WELDER MOVED APPROX 15' OFF TO THE SIDE WHILE THE OTHER ATTEMPTED TO FREE THE ADA PTER USING A HAMMER. WHILE STRIKING, THE ADAPTER, A CHIP BROKE FREE OFF THE ADAPTER, PRJECTING AND PUNCTURING THE OTHERS LEFT ABDOMAL.
THE EE WAS SPOOLING OUT WELDING LEADS TO DO REPAIRS ON THE BUCKET OF A 2800 SHOVEL. WHILE WALKING WITH THE LEADS, THE EE STEPPED INTO A GROUSER TRACK DEPRESSION TWISTING HIS ANKLE.
WHILE EXITING THE CAB OF VEHICLE, EMPLOYEE STEPPED FROM THE TOP STEP OF THE VEHICLE TO THE GROUND WITH HIS RIGHT FOOT. THE EMPLOYEE WAS USING A 3POINT CONTACT SYSTEM WHILE EXITING HOWEVER, DID NOT UTILIZE THE STEP CLOSET TO THE GROUND. THE EMPLOYEE WAS INCOMPLIANCE WITH COMPANY AND MINE
WHILE DISASSEMBLING THE FUEL TANK SUPPORT BRACKETS FOR TANK REMOVAL, THE EMPLOYEE REMOVED THE LAST BOLT FROM THE STEEL PLATE. THE PLATE DROPPED PINCHING AND LACERATING THE EMPLOYEES LEFT INDEX FINGER. THE EMPLOYEES FINGER WAS PINCHED BETWEEN THE PLATE HE WAS REMOVING AND THE TOP OF THE MOTOR GRADER FRAME.

THREE EES HAD FINISHED PREPPING THE TOP SIDE OF A CONVEYOR BELT IN EFFORT TO REPAIR A DAMAGED AREA. UPON COMPLETION, TWO EES WENT TO RETRIEVE THE REPAIR PRODUCTS FOR THE CABL OF PICKUP TRUCK. UPON RETURNING, ONE EE BEGAN OPENING THE TWIST-OFF CAP OF THE BELZONA 2911 CONDITIONER. UPON CRACKING OPEN, THE CONTENTS BURST FROM CAN SPLASHING HIS FACE AND RUNNING BEHIND SAFETY GLASSES.
AFTER SHUTTING DOWN THE 16G GRADER, THE OPERATORDISMOUNTED THE CAB AREA & WALKED TO THE REAR OF THE MACHINE. THE OPERATOR THEN MOUNTED AT THE RIGHT REAR WALKED ALONG SIDE THE ENGINE COMPARTMEN T FOR THE HOUR METER READING. WHILE WALKING BACKTO DISMOUNT THE OPERATOR LOST HER BALANCE FALLING ONTO THE GROUND & INJURING HER RIGHT WRIST Y
THE EE WAS FUELING A FRONT END LOADER. THE FUEL NOZZLE FAILED TO DISENGAGE. THE EE MANUALLY DISENGAGED THE NOZZLE HANDLE. THE DIESEL FUEL SPLASHED BACK FROM THE LOADER FUEL TANK & ONTO THE EMP LOYEE'S FACE. THE DIESEL FUEL RAN BEHIND THE EE'S SAFETY GLASSES AND INTO BOTH EYES.
I HAD PARKED MY PERSONAL VEHICLE AS GOING TO GO TO THE DRAGLINE. AS I STARTED TO EXIT THE VEHICLE, MY RIGHT HEEL CAUGHT MY RUNNING BOARD. AS I CONTINUED TO STAND UP, MY RIGHT ANKLE ROLLED UNDE R AND POPPED. I CONTINUED BOTH TO AND FROM THE DRAGLINE, THINKING I HAD SPARAINED MY ANKLE.
WHILE WORKING OFF A 6' STEP LADDER, EMPLOYEE WASUSING A 3' PRYBAR IN AN EFFORT TO ALIGN AN AIRT\CYLINDER FOR A DRAG LINE BRAKE. HE WAS PRYING AWAYFROM HIS BODY WHEN THE BAR SLIPPED LOOSE CAUSING HIM TO FALL OFF THE LADDER AND DOWN ONTO A STAIRWAY HEADER. EMPLOYEE SUFFERED A LACERATION TO HIS RIGHT UPPER JAW FACIAL AREA.
A MECHANIC WAS WORKING IN THE REVOLVING FRAME OF A BE 2570 REINSTALLING A SEAL ON A SWING SHAFT. THE MECHANIC WHILE WORKING OFF A LADDER, LOST HIS BALANCE FALLING BACKWARDS FROM A HEIGHT OF AP PROX. 4 FEET. THE MECHANIC STRUCK THE BACK OF HIS HEAD ON A GREASE INJECTOR CAUSING A 3/4" LACERATION REQUIRING 5 STAPLES.
