Excavator was being used to finish crushing. It started on fire. The fire was extinguished with fire extinguisher in machine and onsite within 10 to 15 minutes. Employee received burn to left side/back.

General Manager was performing mechanical work on the #1 conveyor which is under the grizzly. The miner crawled under the 4 foot high 39 inch conveyor and was standing in a 2 foot wide area between the conveyor and the back wall of the dump site. When he attempted to identify which roller was bad he became entangled causing tearing injuries to his left arm.
