A loader operator thought EE had a leak on a 980C loader. EE parked the loader and climbed into the front right fender well to inspect for the leak. As EE was exiting the area, EE dislodged EE's hard hat on the fender, and it fell to the ground. EE bent over and picked up EE's hard hat, raised back up; and struck EE's unprotected head on the fender.

The employee was helping the company electrician inspect the wiring on the pit water pump, he tripped and fell injuring his right hand.
Employee was preparing to work on chute at end of #8 conv. While he was standing on the catwalk a part of the catwalk floor gave way causing him to fall onto the catwalk. While he was falling he grabbed the chute and it cut his left hand requiring 8 stitches.

The employee was injured when he fell from a portable ladder. (Ladder propped against conveyor belt. Fell approx. 5-6', landed on concrete pad. Laceration to head - 4 stitches & hand - 7 stitches. Follow up appt. determined probable concussion.)
The Employee was replacing bushings in the arms of a pit loader for the bucket. As he was driving in the bushing a particle entered into the right eye.

WHEN THE MATERIAL CLEARED THE TOP, THE ROLLSPULLED EE FOOT DOWN WARD MASHING HIS BIG TOE AND 2ND TOE REQUIRED STITCHES.

INJURED WAS DIGGING OUT A STUMP AT THE 73 STOCKPILE AND CUT HIS FINGER ON A FOREIGN OBJECT.
