INJURED WAS HOOKING A CHAIN ON TO A CABLE AND THE EXCAVATOR BUCKET TO PULL THE CABLE OUT OF THE WOODS. THE BUCKET CAUGHT A SMALL TREE AND SLAPPED IT BACKWARDS TOWARD THE INJURED AND STRUCK HIS HARD HAT.

CLEANING SNOW OUT OF DUMP TRAILER. WHILE PUSHING SNOW WITH SHOVEL OUT OF DUMP TRAIER, WALKED INTO CROSS BAR HITTING ACROSS NOSE AND FELL BACKWARD, HITTING BACK AND HEAD ON TRAILER FLOOR.

WHILE RAISING TRAILER BED TO DUMP LOAD OF COAL INTO BIN, BED HOIST BROKE, TRAILER HOIST WAS INTO 5TH STAGE NEAR TOP. WHEN HOIST BROKE, TRAILER BED CAME DOWN, BOTTOMING OUT ON TRAILER FRAME, JO LTING TRACTOR CAB WHILE DRIVER WAS INSIDE CAB.

EE STATED HE WAS GETTING OUT OF BACK OF TRUCK WHEN HIS FOOT SLIPPED OFF BUMPER & WHEN HE FELL, HE REACHED BACK TO BRACE HIMSELF. HIS BODY WEIGHT FELL ON WRIST, CAUSING INJURY.
