Daley's Pit · Fall onto or against objects

Mechanic doing maintenance on excavator - finished and was climbing down ladder and slipped off rung and leg hit machine and caused a bruise which caused a blood clot. Grease on rubg of ladder caused him to slip.

CONVEYOR ON WASH-SEPERATOR PLANT STOPPED. EE LIFTED GUARD TO CHECK-WITHOUT SHUTTING OFF POWER. HE TOUCHED CONVEYOR IT STARTED CATCHING TIP OF RIGHT HAND INDEX FINGER CRUSHED TIP OF FINGER.

TWO EMPLOYEES WERE LIFTING FORK FROM WELDING TABLE. OVER-HEAD CHAIN HOIST WAS SUPPOSED TO BE USED FOR LIFTING. WHEN SETTING FORK ON GROUND, FINGERS GOT CAUGHT UNDER FORK.
