Employee was cleaning near a tail pulley using a shovel. Employee was not wearing a hard hat at the time. Employee was bent over and when they stood up, they struck their head on a conveyor cross brace.

Papich Construction Company, Inc. · Fall from ladders

Laborer slipped when descending from conducting drop box maintenance on a 1 1/2 in stacker. Suffered a bad ankle sprain from landing on the ground. No one witnessed the incident. Laborer reported missing foot hold to supervisor, but told doctor at clinic employee slipped from a ladder.

Miner was fueling his Dozer from the Lube truck. When reaching for the nozzle at the completion of fueling, miner felt a pain in his chest. Determined that miner had strained a chest muscle.

Employee says he injured himself on 7/21/15 while attempting to lift a gear box. He said he continued to work "thinking nothing of it." He did not notify us until a week later. Doctor diagnosed him with a strain and performed no treatment, but asked that he return in a week. Last doctor's report on 7/29/15 showed he may have a possible strain.

DOWN ON KNEES DOING CLEAN-UP, STOOD UPRIGHT KNEEPOPPED. AFTER GOING HOME KNEE BEGAN TO HURT. WENT TO HOSPITAL, TREATED & RELEASED. WENT TO DOCTOR 10/31/01 WHO RESTRICTED FROM WORK UNTIL SCHEDU LED MRI. EE WAS RELEASED FOR WORK 10/15 THROUGH 10/31. WORKED 10/24 THROUGH 10/26.
