OPERATOR WAS ON STOCKPILE TO DUMP LOAD. AIR LINE SEEMED TO BREAK AND HE DID NOT HAVE IT IN GEAR. NO AIR- RAN TRUCK OFF SLOPE- HEAD FIRST INTO PUGMILL BINS.
EMPL WAS USING PRESS TO REMOVE BEARING FROM SHAFT. HE WAS HOLDING SHAFT WITH HAND WHEN BEARING RELEASED SHAFT DROPPED. EMPLS LITTLE FINGER WAS PINCHED BETWEEN SHAFT NUT& BEARING. WENT TO ER 4 STITCHES LITTLE FINGER.
