Arcilla Mining & Land, LLC · Struck against a moving object

Miner was operating bulldozer went over a rise on the terrain. As EE came down on the other side, the dozer shift up causing EE to strike the roof due to not wearing seatbelt. Sprained neck taking to emergency room for first aid.

EE injured his back while operating 30 ton Volvo off road truck. Truck was loaded while traveling along bumpy road. EE failed to adjust seat spring tension on seat at beginning of shift to except his weight 240 lbs when truck crossed rough road the seat bottomed out causing EE to hit very hard on floor causing injury.

DRILLER AND DRILL HELPER WERE DRILLING FOR CLAY. DRILL HELPER COUPLED 1 1/4" HIGH PRESSURE WATERHOSE TO CORE BARRELL TO DISCHARGE CLAY SAMPLE. WHEN COUPLED, PRESSURE WAS APPLIED TO THE HOSE CA USING HOSE TO COME LOOSE AND STRIKING DRILL HELPER ON RIGHT SIDE OF FACE CAUSING LACERATION REQUIRING TWENTY TWO STITCHES.
