Customer PU had just left the scale house with a load of rock & headed out the north gate. About 100 yards from the gate the customer noticed smoke coming out from under the engine compartment. Customer stopped & saw fire under the hood. Customer called fire department. No injuries just property damage. Customer left scale house 12:36pm & fire was extinguished around 1pm.

Employee dismounting haul truck did not maintain 3 points of contact. Employee let go with left hand and swung out away from the truck with left foot. Fell to ground straining left ankle.

Employee was using high pressure water hose to wash out a pan feeder when the pressure of the hose caused the nozzle to flip back and strike employee above R-eye socket.

Technicians were in the process of removing the transmission on a 988G. They had installed 4 lifting links with bolts. The chain being used was a four legged spreader chain. While pulling on chains with the boom, the lifting bolts were pulled out of their threaded holes allowing the chain, which was under tension to flip up and hit the technician in the face.

Driving stockpile truck and noticed something on right forearm and swatted it. Doctor stated employee was bitten by a spider.

No injuries. Employees were changing bearing on #3 conveyor. Sparks from torch fell to the lowest deck on the scalper screen. Screen deck was rubber and caught fire. Fire was extinguised but started again and and got bigger. The fire department was called to put the fire out.

An employee with 2 years mining exp & 1 week at Sieboldt was operating a stockpile truck at the plant. When employee opened door of truck his left hand made contact with a roll pin that attached the handle to the door. The pin head was mushroomed and caused a small laceration. Employee received 3 stitches and was released with no work restrictions.

EMP CLAIMS HE BECAME ILL WHILE HANDLING AN/FO. DR SCHEDULED HIM TO HAVE FURTHER TESTS WITH SPECIALIST AND EMP REFUSED TO GO. EMP WAS SCHEDULED TO WORK AFTER BEING RELEASED BY DR. FAILED TO REP ORT TO WORK AND WAS TERMINATED.
WHILE LOADING A SHOT ON ROUGH TERRAIN, THE EMPLOYEE STEPPED ON A ROCK AND TWISTED HIS ANKLE THE ACCIDENT HAPPENED ON 9-24-96 BUT WAS NOT REPORTED TO US UNTIL 10-16-96.
