EE CHANGED GEARBOX IN CRUSHER ON 5-10-01. NEXT DAY HE FELT A PAIN IN HIS ABDOMEN, WHICH CONTINUED FOR 4 DAYS. HE DID NOT REPORT IT THEN. HIS LOWER ABDOMEN BEGAN HURTING AGAIN ON 6-10-01 WHEN H E WAS WORKING IN THE PLANT, AND HE REPORTED PAIN WAS SEEN BY DR, PRESCRIBED ANTI-INFLAMMATORY & MUSCLE RELAXANT AND PLACE ON LIGHT DUTY. *NOT REPORTED TO EMPLOYER UNTIL 6-10-2001.

TYRING TO POSITION A CULVERT THAT WAS BEING LOWERED INTO PLACE BY A FRONT END LOADER AND A CABLE. HE WAS PUSHING ON THE CULVERT & STRAINED HIS BACK. HE DID NOT LOSE ANY TIME UNTIL 1-13-88 (HE THEN LOST 10 DAYS).
