Annual hearing tests were performed on 2/14/2023. It was documented that employee's hearing has dropped below the threshold of EE's original baseline. On 2/20/2023, EE was sent to an audiologist to be retested in a clinic setting and the results were the same. EE is being referred to an ENT doctor to ensure the hearing loss is not due to a medical condition.

EE was performing start of shift maintenance on KPI 4250 (Rock Crusher) attempting to add anti-freeze to the radiator the EE fell approx 5ft from the position where they were standing to the crusher deck. EE was treated in the local ER diagnosed with a right AC joint separation, Non displacement fracture to C4, and two broken ribs. The EE was treated and released from the ER.

A Leak was found in a 500 gal propane tank outside the office building. All personal were evacuated to the quarry main gate. The gate is about 1/4 mile from the office. Once the propane company had check and repaired the tank the all clear was given.

EE was exiting heavy equipment (rock breaker Komatsu 200) at the end of his shift on Tuesday May 22nd around 6:30p.m. EE stepped from the tread of the equipment onto the step when the step broke causing the EE to contact the tread/broken step with his lower right leg. EE did not report injury to supervision until Wednesday May 30th.

Primary crusher truck dump bridged and had to throw chain under/over large rocks to break bridge. Employee proceeded to climb into the hopper to break the jam, while doing so employee stood on a rock instead of a flat surface. The rock slipped out from under the employee which caused the employee to fall striking his tailbone on a rock. Employee suffered a bruised tailbone.

WHILE PUTTING A NEW VBELT ON THE CRUSHER, EE WASUSING A SCREWDRIVER TO ROLL THE BELT FROM ONE GROVE TO THE NEXT TO GET IT IN ALIGNMENT ON THE PULLEYE. THE SCREWDRIVER FLIPPED OUT WHEN EE ROL LED THE BELT STRIKING HIM ON THE CHIN CAUSING A LACERATION REQUIRING 6 STITCHES
