During a belt adjustment on a screening plant, a miner's hand was caught between the counterbalance and the belt. Initially treated with first aid, no fracture was detected. However, a subsequent doctor's visit revealed a fracture, making the injury reportable and explaining the delay in reporting beyond the 14-day window.
The miner felt a pull and sharp pain in EE's right lower back while climbing onto an excavator, along with pain in EE's upper left shoulder and neck. Initially treated with first aid, the injury became reportable after a subsequent doctor's visit led to a referral for physical therapy, explaining the delay in reporting beyond the 14-day window.
The employee was tac welding on a shop table when the incident occurred. During the night the employee had pain in eye. The employee went to the ER for treatment and was given prescription eye drops. The employee returned to work the next morning without pain and had no missed time.
Miner was unfolding a stacker conveyor when EE looked up and was hit in the face with a small falling rock. ER visit confirmed facial fracture.
Mechanic was working with a wrench to straighten a piece of metal. While pulling the wrench, they got too close to the handle and the wrench slipped, striking their forehead. Mechanic was given 5 stitches to close their head wound. No LOC.

Independent contractor was positioning a screening plant in place when left middle finger was pinched between two pieces of metal on the cat walk. The contractor received 4 stitches. There was no amputation or loss of function in the finger/hand.

Miner was clearing material beneath the hopper for the stacker belt next to the screening plant when a rock jumped out of the hopper and hit Miner's hard hat.

The miner was climbing down off a machine when they lost grip and fell a few feet to the ground. Employee sprained an ankle when they fell
Miner was exiting the Motorgrader when a finger got caught between the door and the machine. The tip of the 4th finger on left hand was found to be fractured.
Employee was moving a 4" pump when they tripped over a hose. Employee used left arm to catch their fall, injuring their left shoulder.

A miner reached into the head pulley area to dislodge a rock. The miner's right arm caught in the belt and pulled the miner up and over the top of the belt. A nearby miner immediately hit the E-stop. The supervisor was radioed immediately and cut the belt to release the miner. A tourniquet was placed on the miner's arm and an ambulance transported the miner to a nearby hospital.
Miners, #1 and #2 hosted a cookout with beer in our Shop parking lot after work hours. Seemingly distraught over a car backing accident, Miner #1 climbed down a highwall of an adjacent sand pit full of water, with Miner #2 following and explaining everything would be okay. Miner #1 went into the water and Miner #2 followed in after them. Both drowned.

Stepped off of excavator track onto lose materials and pivoted. He then heard a pop and felt a pain like a needle in his right knee. Then next morning, his knee was swollen.
Helping to fuel screener 2100, helping with hose, slipped on mound of muddy material and hard hat came off. When he raised up, he hit his head on a cross support of the screener. Taken to Texas Health Harris Methodist Hospital where he received 6 staples in the top of his head for the laceration. No lost time at work.

Excavator 20 feet from a 20-foot face of soil pulling material down. Moisture content of wet soil caused soil to fall in a clod instead of what we generally have observed streaming down loosely. 1 clod rolled up track of excavator, breaking windshield, & pushing it to our operator, not injured except for minor reported scratch on his chest. Windshield broken, ROPS good per CAT.
Digging rock debris from underneath the hopper of a conveyor belt. Pushed shovel into pile of rock and felt a small pop and then a sharp pain went through his right arm.

A contractor (Reeder Oil) was delivering a 500 gallon hydraulic oil storage tank on a tilt bed delivery truck. EE and another ee began assisting the contractor in unloading the tank. While the tank was being unloaded the tilt bed on the truck was lowered striking ee left foot fracturing his toe.
