Skyline Dredging & Excavating · Fall onto or against objects

EE was performing a normal pre shift inspection on the cone crusher. He checked the oil and stepped up onto the belt guard to inspect the cone. When he attempted to dismount the guard his foot slipped, he was unable to hold his weight with his hands and he fell on to the floor and handrail, bumping his head either on top/bottom rail. He was diagnosed with concussion.

Skyline Dredging & Excavating · Fall to the walkway or working surface

The #1 screen at jobsite had plugged and was overflowing material. In an effort to get it stopped, EE took off running towards control panel to shut it off. EE tripped and fell to the ground, landed on right shoulder and broke clavicle. Went to hospital, treated and discharged. Advised to take days off til 1/17/12 and light duty, no lifting over 10lbs until 1/27/12.

Skyline Dredging & Excavating · Struck by falling object

EE was between the light plant and the screen when a rock approx 6" in diameter fell from atop the screen and struck EE on hard hat, rendering the EE unconscious. Supervisor sent EE to hospital. Had CT Scan, evaluated by a doctor and diagnosed w/concussion and discharged w/restrictions of: Don't put yourself at risk of another concussion until symptoms of this one subside.
