Smart Sand Blair LLC · Struck by falling object

Employee was working with another employee to stack Rotex screen decks when a stack of screen decks on the ground next to them tipped over, contacting their right ankle. The employee was evaluated with an ankle contusion and given light duty restrictions. Employee has not missed any shifts.

On 1/10/26, EE was cleaning a plugged chute and experienced pain in EE's right shoulder. EE evaluated by doctor immediately and was not given any work restrictions that prevented EE from completing EE's normal job duties. On a follow-up appointment on 1/29/26, EE was issued a restriction to be off work.

While dismounting from a portable ladder and area during replacement of a conveyor counterweight, an object fell striking miner in the head requiring sutures in two places. Employee was cleared to return to work without any restrictions.

Employee was washing out elevated areas with a high-pressure hose and material fell striking the miner. Employee received a non-conclusive diagnosis of a possible concussion but out of caution was given work restrictions for return as if EE did receive a concussion.

Employee was using a bar to pry on a scraper. Bar slipped and employee felt a pop in right shoulder. Employee reported the incident and said that the shoulder was sore but fine and did not want to seek medical treatment. 2 and 2 days weeks later on 2-13-20 employee decided to seek treatment. No clear diagnosis was given, but employee did receive restrictions.

Employee felt discomfort in eye on 1/6/2020 after repeated flushing with no relief on 1/7/2020 employee reported discomfort to supervisor. Employee was seen at eye clinic where a small piece of metal was removed from the eye.

Employee was walking on top of a rail car when fall protection device became snagged on the supporting structure. This caused employee to be stopped mid stride while stepping from one car to the next. This caused employee to lose balance and fall between the cars. Total fall before arrest by retractable lanyard was approximately 4 feet.

The employee stated that ee was looking around the plant when this happened and not fully focused on the stairs. It was dark out and ee may have cast a shadow over the bottom stair as ee descended.

Employee was attempting to get a strap around a broken blow bar when the bar dislodged from the backing and pinched left thumb against a rotor. EE went to have an X-Ray and shows a potential hairline fracture of thumb.

EE began having chest pain at 9p after climbing stairs that went away. At 11p more pain came after taking stairs to top of silo. EE came off silo and sat in control room to rest. EE was unsure what was causing the pain, but was told to go in by co workers. EE drove to ER and there was diagnosed with a heart attack. Stint was placed. EE stated that DR said EE had 99% blockage.
