EMPLOYEE WAS FEEDING MATERIAL TO CRUSHER WITH LOADER. AS HE WAS READY TO DUMP A LOAD OF MATERIAL INTO THE CRUSHER, THE BACK END OF THE LOADER RAISED UP AND SWUNG AROUND. THE MATERIAL BERM GAVE WAY AND THE LOADER ROLLED DOWN THE HILL LANDING UPSIDE DOWN. EMPLOYEE RECEIVED SLIGHT SCRAPES AND BRUISES.

EMPLOYEE WAS PUTTING NEW LAGGING ON SHUT OFF CONVEYOR TO IMPACT CRUSHER. AS HE STRAIGHTENED UP TO GET OFF THE CONVEYOR HE FELT A SHARP PAIN IN HIS BACK.
EMPLOYEE WAS REMOVING COMPENSATOR FROM CRUSHER, USING A HAMMER TO DISLODGE A NUT. THE HAMMER DEFLECTED OFF THE RIM OF THE CONE, CAME DOWN STRIKING EMPLOYEE ON LEFT HAND, BRAKING HAND.
EMPLOYEE WAS LOOSENING CAP OVER AIR POT (SPRING LOADED) TO CHANGE RUBBER DIAPHRAM ON 175 MICHIGAN LAODER, SPRING RELEASED CAP WHICH HURLED UP STRIKING EMPLOYEE ABOVE RIGHT EYE BROW. EMPLOYEE RQUIRED STITCHES TO GASH IN HEAD ABOVE RIGHT EYE.
