EE HAD A UTILITY KNIFE TAPED TO AN ALL THREAD END USING IT TO CUT A PIECE OF BELTING OUT OF A PUMP SUCTION PIPE. HE TRIED TO TAKE THE ALL THREAD & UTILITY KNIFE LOSE BY PULLING THEM APART WHEN HAND SLIPPED ACROSS KNIFE & CUT FINGER.
EE WAS INSTALLING A PLANITARY GEAR HOUSING ON A CAT 980C HIGHLIFT WHEN HE FELT A PAIN IN HIS LOWER BACK ON RIGHT SIDE. HE FINISED THE SHIFT AND WORKED THE NEXT SCHEDULED SHIFT, SO THERE WAS O IMMEDIATE LOST TIME. HE TOOK A PERSONNEL DAY ON MAY 11,1999, THAT WAS THE ONLY DAY HE HAD MISSED UNTIL TODAY. HE HAD A DISPUT WITH HIS SUPERVISOR ON 6-11-99 & THEN DECIDED TO SEE A DR.
