2 recordedTHE VICTIM WAS BUILDING A BERM AROUND THE OUTER EDGE OF THE PIT. HE WAS OPERATING A 1977 HOUGH MODEL H-80 FRONT-END LOADER WITH A FOUR YARD BUCKET. HE DROVE THE LOADER OVER THE EDGE AND ROLLED DOWN A 30 FT BANK. HE WAS NOT WEARING HIS SEAT BELT.

Loader operator made wrong adjustment on loader seat which bottomed out causing lower back injury.
We are contesting this accident happened at work. All co workers say itwas a normal routine day. EE did not complain of pain and limp at any time and he did not report any accident to supervisor. He called he went home & middle of night he got shooting pain in his knee. spent a lot of time on his knees at work and he must have over exerted his knee.

Employee was welding in an enclosed area and he was wearing his welding mask. He worked all day and when he went home and tried to sleep his eyes hurt. He then went to St. Mary's hospital to get some drops for his eyes. He was fine the next day and worked the full day.
Employee was running to turn off feeder, when she stepped over a mound of dirt & overextended her calf muscle.

Employee was working on the feed hopper using a porta-power unit to straighten the side of the hopper when porta-power slipped and a 12" x 14" x 3/8" steel plate hit him in the face and chest.
EE WAS ARGUING WITH THE SHOP FOREMAN. THE FOREMAN THREW AN EMPTY GREASE GUN ABOUT 15 FEET AWAY FROM EE. WITNESSES CLAIM EE HAD GREASY HANDS AND A GREASE RAG WHICH HE WIPED HIS EYES WITH. SINCE THERE WAS A REFRIGERATOR BETWEEN EE AND THE GREASE GUN, THERE IS NO WAY GREASE SPLATTERED ON EE FROM THE GREASE GUN.

WHILE EMPLOYEE WAS ARCH-WELDING A PIECE OF EQUIPMENT, A PIECE OF SLAG FELL DOWN INTO HIS BOOT, BURNING HIS FOOT. THIS REPORT LATE BECAUSE EE WORKED A FULL WEEK AFTER ACCIDENT AND NEVER BROUGHT ANY DOCUMENTATION IN UNTIL 8/10/04.
