Employee was helping a co-worker work on a tail pulley. Employee was turning a threaded rod while a co-worker was hitting a bearing with hammer, bearing came loose suddenly and the treaded rod took skin off EE's fingers.

Due to weather was told to stay home on 1/7/14. Employee called in on 1/7/14 said he was going to ER due to the incident that happened on 12/5/13. He received no medical treatment and no lost time till 1/7/14

EE WAS ATTEMPTING TO GET AN OVERSIZE ROCK THAT WAS HUNG IN THE PLANT SURGE TUNNEL HE SET A LADDER UP BESIDE THE FEEDER & PROCEEDED TO CLIMB UP THE LADDER WITH A 12PD SLEDGE HAMMER IN HAND. HE WAS GOING TO TRY & HIT THE FEEDER BOX TO JAR THEROCK LOOSE. WHEN THE EE GOT UP ON THE 4TH STEP OF THE LADDER & SWUNG THE HAMMER BACK THE LADDERLEG BROKE & THE LADDER COLLAPSED.

SUBECT LOST CONTROL OF THE GUARD AND THE GUARD FELL INTO THE PUGMILL WHERE THE AUGE RS WERE TURNING. THE PADDLES ON TEH AUGEERS CAUGHT THE GUARD AND PROPELLED IT UPWARD INTO THE VICTIMS CHIN.

EMPLOYEE WAS OPENING THE LEFT DOOR OTN EH 50 TON EUCLIDE HAUL UNIT FOR ACCESS TO THE OPERATOR SEAT. EMPLOYEE LEANED DOWN TO THEDOOR HANDLE AND PULLED HTE DOOR OPEN THE TOP CORNER OF THE DOOR STRUCK THEEMPLOYEE IN THE LEFT SIDE OF HIS CHIN AND BROKE THE CAP OFF OF A TOOTH. THIS EMLOYEE HAD JUST LEFT A 30 MINUTE SAFETY MEETING ON HAZARD RECOGNITION MINUTES PRIOR TO INJURTY.

PUMP IS HEAVY & HAS TO BE MANOVERED BY HAND TO ALIGN BOLT HOLES EMPL CLAIMS HE FELT PAIN IN RIGHT SHOULDER HE REPORTED INJ 01/02/91.

HE WAS HELPING LOAD A SHOT WHEN ROCK KEPT FALLING INTO THE HOLE. HE WAS GETTING THE ROCK OUT OF THE HOLE WHEN A SMALL ROCK FELL AND HIT HIM ON THE MIDDLE KNUCKLE OF THE RIGHT HAND KNOCKING THE SKING OFF. THE SCRATCH THAT WAS NOT TAKEN CARE OF BECAME INFECTED AND THE INJURED WAS HOSPTIALIZED.
