Employee was on platform waiting for skid steer to pull pipe away from building structure when opposite end of pipe flipped up and caught employees finger between pipe and handrail causing a laceration to right middle finger

Injured employee was helping another employee that was using pry bar on toggle plate when Injured employee hand was pinched between thumb & back of hand. On 8/10/2020 Injured employee reported that hand was hurting needed to go clinic get checked out. On 8/24/2020 Dr put splint on.

EE was installing battery in Haul Truck when EE reached down to pick battery up EE stated they strained back. Did not go to Doctor & no lost work days. Decided to go to Doctor in April no loss days. Went back May 6 Doctor put EE on a modified duty but with restrictions, we had no job so EE is now out of work til next Dr. Appt on May 16.

Employee is our QC/QA Testing Technician. He picked up a 5 gal bucket of material to dump out when he felt pain in his lower back and leg.

Employee climbed into loader to start up then dismounted to perform pre-shift. When he stepped down from bottom step he turned to go round front of loader when he felt pain in lower back causing a sprain lumbar

Employee was unclogging drain on sand screw with a pipe. His left little finger struck grate on top of sand screw causing a laceration that required stitches.

While using air bags to dislodge large blocks of rock, the scaler's legs were taken out when rock fell & air bags went down the hill. Dragged scaler into area that rock was falling.

EE strained lower back while lifting a 55 gallon drum of oil.

Lacerated left pinky finger after the WA500 Loader cylinder pin pinched it, receiving 10 stitches.

LOADER DROPPED BIG ROCK ON HAUL TRUCK. ROCK WAS BIG ENOUGH TO JERK DRIVER OF HAUL TRUCK AROUND & INJURE HIS BACK.

DAVID WILSON AND HAROLD HAMLETT WERE CLOSING DOORSA ON BALLAST CARS. CAR SOU 994354 CAME IN WITH DOOR HALF CLOSED AND ROCKS WEDGED IN DOOR SO IT COULD NOT BE CLOSED WITHOUT REOPENING DOOR. A FTER DOOR WAS OPENED AND CLEANED, DAVID AND HAROLD TRIED TO CLOSE THE DOOR. THE DOOR WAS BENT WHERE IT PASSED THE LATCH AND WAS HANGING TO LATCH. D & H WERE ON OPPOSITE SIDE OF THE CAR, DAVID

EE WAS STANDING ON GROUND CLOSING DOOR ON N-S HOPPER CAR USING A 5 FT LEVERAGE BAR. THE LATCH HAD A SPACER PLATE WELDED ON IT6. THIS CAUSED EXCESSIVE LEVERAGE TO BE USED TO CLOSE DOOR. THE LA TCH SLIPPED CAUSING THE BAR TO SLIP AND THE FALLING BACKWARD TO GROUND.

THE EE WAS CLOSING DOORS ON RAILROAD BALLAST CARS. THIS IS DONE BY APPLYING DOWNWARD PRESSURE ON A RATCHET ON THE DOOR. THE EE WAS STANDING ON THE GROUND PUTTING DOWNWARD PRESSURE ON THE RATCH ET WHEN THE RATCHET SLIPPED CAUSING HIM TO STRIKE A BAR ON THE SIDE OF THE CAR WITH DOWNWARD FORCE. THIS BAR CAUGHT HIS PRESCRIPTION SAFETY GLASSES CAUSING THE BOTTOM OF THE FRAME BELOW LENS.

WHILE CLEANING UP SCRAP STEEL OUT OF RAIL CARS,EMPL BENT OVER FROM WAIST TO PICK UP END OF 130 LB STEEL BAR.WHEN HE GOT UP HE HAD PULLED SOMETHING IN HIS BACK.

A BLOCK OF WOOD USED TO SUPPORT TRUCK BED FELL OFF TIRE STRIKING INJURED IN THE LEFT HIP

EMPLOYEE WAS OPERATING FRONT END LOADER WHEN HE SUFFERED WHAT APPEARED TO BE A HEART ATTACK.

WELDER WAS LINING FEEDER FOR TURUNNEL BELT AT THE PLANT HE HAD WELDED THREE STEPS ON SIDE OF FEEDER TO GET IN & OUT WITH THE STEEL USED TO LINE THE FEEDER WHILE BRINGING SOME STEEL INTO THE F EDER HE SLIPPED ON SOME LOOSE MATERIAL ON THE TOP STEP FELL & HIT HIS LEG ON THE STEP CUTTING HIS RIGHT UPPER LEG

HIT BOLT WITH CHIPPING HAMMER FRAGMENT. OFF CHIPPING HAMMER CAME OFF AND EMBEDDED IN BACK OF LEFT HAND.

MR MYERS WAS STANDING BESIDE MR EPPS WHILE MR EPPS WAS GRINDING ON NUT AT CONE CRUSHER A PIECE OF METAL FROM THE NUT EMBEDDED IN MR MYERS LEFT EYE
