Employee was in shop getting metal for plant, they walked around back of truck hitting right shin on hitch could not maintain balance and fell to ground causing a fracture to left femur.

Injured employee was helping another employee that was using pry bar on toggle plate when Injured employee hand was pinched between thumb & back of hand. On 8/10/2020 Injured employee reported that hand was hurting needed to go clinic get checked out. On 8/24/2020 Dr put splint on.

EE was installing battery in Haul Truck when EE reached down to pick battery up EE stated they strained back. Did not go to Doctor & no lost work days. Decided to go to Doctor in April no loss days. Went back May 6 Doctor put EE on a modified duty but with restrictions, we had no job so EE is now out of work til next Dr. Appt on May 16.

EE WAS STANDING ON GROUND CLOSING DOOR ON N-S HOPPER CAR USING A 5 FT LEVERAGE BAR. THE LATCH HAD A SPACER PLATE WELDED ON IT6. THIS CAUSED EXCESSIVE LEVERAGE TO BE USED TO CLOSE DOOR. THE LA TCH SLIPPED CAUSING THE BAR TO SLIP AND THE FALLING BACKWARD TO GROUND.

THE EE WAS CLOSING DOORS ON RAILROAD BALLAST CARS. THIS IS DONE BY APPLYING DOWNWARD PRESSURE ON A RATCHET ON THE DOOR. THE EE WAS STANDING ON THE GROUND PUTTING DOWNWARD PRESSURE ON THE RATCH ET WHEN THE RATCHET SLIPPED CAUSING HIM TO STRIKE A BAR ON THE SIDE OF THE CAR WITH DOWNWARD FORCE. THIS BAR CAUGHT HIS PRESCRIPTION SAFETY GLASSES CAUSING THE BOTTOM OF THE FRAME BELOW LENS.
