On 04/16/16, The EE was welding, a spark flew back from a burning torch, it burned through EE's jeans causing it to burn EE's knee. On 04/17/16 EE had to go to a clinic, they sent EE to ER at HCMC, where they gave EE an antibiotic. EE returned to full time duty 04/21/16.

PULLING ON A TRAY IN THE ROCK SHAKER UNIT, COULD NOT REMOVE THE TRAY SO HE PULLED AT IT MORE AND INJURED HIS LEFT THUMB

IW was with another worker, on a belt attempting to rotate a gear box down. As they were rotating, the head pulley went into rotation causing the head pulley to turn and it moved the belt, causing the worker to get trapped on the belt and was wedged in between the belt causing his body to get twisted injuring his right side neck, shoulder, back, hips.

Employee was assisting crew in installing a conveyor belt. A rope was tied to the metal lacing. The rope was run under the rollers and attached to a truck. As the truck pulled the belt up, the rope broke. The metal belt lacing struck the employee in the right hand resulting in a soft tissue contusion and small hairline fracture.
