On 04/16/16, The EE was welding, a spark flew back from a burning torch, it burned through EE's jeans causing it to burn EE's knee. On 04/17/16 EE had to go to a clinic, they sent EE to ER at HCMC, where they gave EE an antibiotic. EE returned to full time duty 04/21/16.

PULLING ON A TRAY IN THE ROCK SHAKER UNIT, COULD NOT REMOVE THE TRAY SO HE PULLED AT IT MORE AND INJURED HIS LEFT THUMB

IW was with another worker, on a belt attempting to rotate a gear box down. As they were rotating, the head pulley went into rotation causing the head pulley to turn and it moved the belt, causing the worker to get trapped on the belt and was wedged in between the belt causing his body to get twisted injuring his right side neck, shoulder, back, hips.

Employee was lifting a pipe that was 6' long, 2" diameter, and approximately 40 lbs.

Employee was assisting crew in installing a conveyor belt. A rope was tied to the metal lacing. The rope was run under the rollers and attached to a truck. As the truck pulled the belt up, the rope broke. The metal belt lacing struck the employee in the right hand resulting in a soft tissue contusion and small hairline fracture.

Employee was using a hammer to clear material build-up away from wing rubber that seats against conveyor belt. The belt caught the hammer and pulled his left hand between the wing rubber and the belt. Accident resulted in denudation of the left wrist and part of left hand.

LYING ON HIS BACK PERFORMING MAINTENANCE ON THE SCREENS WHEN A PIECE OF DEBRIS FELL INTO HIS RIGHT EYE.

EE WAS PULLING A WATER HOSE, TRIPPED, HOSE FLEW BACK AND HIT HIM IN THE BACK. WAS SORE THE NEXT MORNING. WENT TO DOCTOR TO HAVE IT CHECKED OUT. NO SERIOUS INJURY. TOLD TO APPLY HEAT AND TAKE M OTRIN.

EE WAS WELDING. SAYS SPARKS BURNED HOLE IN OXYGEN LINE & HIS JACKET CAUSING FLAMES UP SLEEVE OF JACKET & BURNING HIS ARM.

WHILE INSTALLING A TRAVEL MOTOR IN GEARBOX OF DRAGLINE, LIFTED MOTOR AND STRAINED BACK.

WHILE WORKIN GON TAIL PULLEY OF #1 CONVEYRO IT SLIPPED, HE LOST BALANCE & FELL INTO OLD SCREEN WHICH STUCK SMALL HOLE IN CALF OF LEFT LEG

SLIPPEFD ON TARP STAND WHILE CLEANING TRUCK RAILS--FELL BETWEEN STAND & TRUCK TO THE GROUND.

STEPPED OUT OF THE PLANT ONTO THE STEPS AND STEP BROKE. STATED HIP AND BACK HURT. HAS SMALL CUT ON LEFT THUMB.

A CABLE HIT HIM IN THE RIBS AFTER HE UNHOOKED IT FROM CONVEYOR AND RELEASED IT.

WAS CUTTING CONVEYOR BELT & THE KNIFE SLIPPED OUT OF THE BELT & INTO HIS UPPR RIGHT LEG.

HE WAS INSIDE GREENFEEDER TO SO SOME WELDING WHEN HE TWISTED HIS BACK.

BURR ON DRAGLINE GEAR THAT HE WAS WORKING ON SLIPPED AND CUT BACK OF HAND. 1 1/2" CUT ON BACK OF RIGHT HAND. RECEIVED 10 STITCHES.

CLIMBING LADDER GOING UP TO SCREW PLATFORM TO FOREMAN DID NOT REPORTA UNTIL 5-4-93.

RELINING CHUTE FOOT SLIPPED WHILE OTHERS WERE MOVING A PIECE OF AR PLATE DID NOT FALL

HE WAS USING SHOVEL TO CLEAN OUT FROM UNDER TAILPULLEY OF CONVEYOR. HIS FEET SLIPPED AND HE SLID DOWN HILL.

WORKING ON CONVEYOR BELT AND JUMPED TO THE GROUND TWISTED RIGHT ANKLE.

HE WAS CHANGING ANUILS IN IMPACT CRUSHER HIT ANUIL WITH HAMMER TO LOOSEN IT A PIECE OF ANUIL CHIPPED OFF AND HIT HIS FACE ONT HE R CHEEK AND IMBEDDED IN FLESH

SITTING ON SIDE OF HOPPER, BENT DOWN PRATICALLY UPSIDE DOWN WHILE WELDING BACK STARTED HURTING POSSIBLE SLIPPED DISC.

LIGHTED TOUCH FELL OUT OF WELDER. HAND INTO LEFT GLOVE BURNING LOWER LEFT ARM.
