Trying to dislodge a rock 17' down a bore hole with loading poles. The rock was dislodged and the poles rebounded and hit employee under the right eyebrow. Cutting EE with a 1' cut. EE was wearing safety glasses. EE was taken to the emergency care unit. for stitches.
8/6/2024, EE experienced unspecified back pain while operating haul truck. 8/7/2024, EE was seen at clinic, first aid only. 8/22/2024, EE condition did not improve, occupational doctor prescribed physical therapy.

Employee was attempting to dislodge a rock that was caught between the frame and arm of the #1 cone crusher. Employee grabbed a chipping hammer and hit the rock, attempting to break it, as EE contacted the rock, debris went underneath EE's safety glasses and hit EE's right eye.

Employee was shoveling around the locked out tail pulley. EE climbed between the trusses of the conveyor and shoveled toward the tail pulley. The belt rolled backwards and EE placed both arms toward the tail pully, placing arms between the tail pulley and belt. The belt continued to roll backwards pinning EE's arms between the tail pulley and belt.

Employee was conducting maintenance work on a guard when at some point debris made contact with employee's eye. Employee was wearing all the required PPE including safety glasses. Towards the end of the shift employee attempted to flush out the eye & thought it was improved. On 01/01/21, employee reported that the eye was red & needed to be seen by a medical professional.

EE was changing a cutting edge on a 982 loader. The bolt head was spinning while the EE attempted to remove the nut with an impact wrench. EE placed their right hand under the head of the bolt to push it up and hold it in place. As the pulled the trigger on the impact, the bolt spun causing a laceration to their right hand. The EE was wearing gloves.
EE was shoveling material into 5 gallon buckets, weighing 80 lbs, then carried one bucket in each hand 30 feet upon where the EE dumped the material on to a belt. The EE experienced a strained muscle in their back right shoulder, due to the heavy lifting and dumping of material on the belt.
EE was asked to put new tape markings on the pump hose 5058. He needed to cut old tape off. In this process, he stabbed himself between the thumb and finger. We took him to the emergency room. He needed four stitches and was released.

While removing worn manganese liner from Crusher Bowl Frame, one employee was driving wedges under line with hammer, the liner popped from it's position, hitting the employee on the lower left rib cage and then on the right foot.
Employee tried to strike a piece of the steering arm of a haul truck in place with a hammer. Employee missed his target and hit his left middle finger. Examination by a doctor revealed a fracture in his finger.

Employee alleges he hit his head on the top of the loader cab on 4/14/04. The injury was not recordable at that time. Employee returned to physician on 5/13/04 claiming that injury was not healing, and was prescribed physical therapy at that time.
Employee reported injury on 11/4/03 (sore shoulder). Recent evaluation led to surgery on 4/6/04.
