January 19, 2007 MT · Metal/Non-Metal bulldozer operator, universal operator, heavy equipment operator, operating engineer HANDLING OF MATERIALS

Worker was assisting in setup of mobile crusher operation by checking feeder pulley to determine cause of slippage. Due to shadows he removed his safety glasses and dust got into his eye.

December 15, 2004 MT · Metal/Non-Metal crane operator, mobile equipment operator (meo), dragline operator, dropball operator, rigger SLIP OR FALL OF PERSON

The miner was climbing down a fixed ladder on a mobile screening plant, when he misplaced footing on the rung, this caused him to lose his hand holds and fall approx 4 ft backwards and twist his right knee. He stated the rung was not icy and that he just misstepped. He had been assisting in screen changing.
