OPERATOR WAS STANDING OUTSIDE THE CAB OF THE DOZER MAKING SOME REPAIRS, WHEN DIRT CAVED IN ON THE MACHINE PUSHING EMP INTO THE MACHINE CAUSING HIM TO CUT HIS FOREHEAD AND TWIST HIS ANKLE.

EMPLOYEE WAS CLEANING COAL OUT OF BOTTOM ROLLER ON CONVEYOR SO HE COULD GREASE THE ROLLER BEARING. WHILE DOING THIS EMPLOYEE GOT HIS HAND STUCK IN THE CONVEYOR BELT AND SMASHED HIS HAND.
