EE DROPPED RIGHT FRONT WHEEL OF FLAT BEDTRUCK OFF RIGHT SHOULDER. IN HIS ATTEMPT TO GET BACK ON THE RAOD HE OVER STEERED AND LOST CONTROL GOING OVER AN EMBANKMENT ON LEFT SIDE OF ROAD
PAD BOX BROKE ON REAR AXLE CAUSING AXLE TO TWIST THUS BREAKING OTHER PAD BOX. THIS RESULTED IN REAR AXLE FALLING OFF TRUCK AND CAUSING LOADED TRUCK TO TURN OVER ON IT'S RIGHT SIDE. DRIVER WAS PINNED IN CAB.
OPERATOR WAS LOADING SPOIL MATERIAL AND ROCK INTO A TRUCK. EE SWUNG THE BUCKET AROUND STRIKING A LARGE ROCK JERKING THE EXCAVATOR AND OPERATOR.
EMPLOYEE WAS PUSHING DIRT AWAY FROM HIGHWALL WHEN THE HIGHWALL FELL ON DOZER. ROCK AND DIRT CAME INTO CAB STRIKING EMPLOYEE.
EE WAS CLOSING LID ON 99SG WHEN HIS LEFT FOOT SLIPPED CAUSING HIM TO FALL HEAD FIRST IN THE HOLE WHERE THE TRANSMISSION IS. THE EE'S KNEE TWISTED SIDEWAYS AND HE LANDED ON HIS ELBOW
