ROCK FALL WAS 4 1/2' THICK 18 X 18 FEET WIDE IN INTERSECTION OF #5 ENTRY 5 BREAKS UP ON #2 BELT INTAKE. SHOWED NO SIGNS OF ACTIVITY BEFOR IT FELL DANGERED OFF AND BUILT CRIBS AROUND IT DOES NOT AFFECT VENT OR TRAVEL WAY
ROOF FALL WAS IN THE #5 ENTRY ON NEUTRAL SEVEN BREAKS UP FROM #1 BELT ON SECTION BELT. WAS CRIBBED OFF & DANGERED OFF. WAS NOT AWARE OF FALL UNTIL EMPLOYMENT OF 1/1/05. THAT SECTION OF MINE HAS BEEN SEALED OFF.
