Sunrise Aggregates LLC · Caught in, under or between running or meshing objects

Being trained in shut down for days end & accessing issues/repairs for next days start up - operator in process of shutting down power. New EE stepped onto conveyor beltline still running, other EE motioned EE to get down & new EE slipped and hit conveyor belt which carried EE down to pinch point trapping EE's foot.

Sunrise Aggregates LLC · Fall to the walkway or working surface

Employee was hoisting a steel beam with a chain. The chain slipped and the steel beam fell approximately 3 ft. Employee was on top of the beam when it fell and he slipped off when it hit the ground, breaking his leg. Owner took employee to the emergency room at the local hospital where he was admitted for treatment.
