A BOLT HOLDING THE BRACKET BROKE CAUSING HIM TO FALL. HE HIT HIS BACK ON A METAL PLATE THAT HOLDS THE DUMP. HE ALSO HIT HIS HEAD ON ANOTHER PLATE.
