Employee was walking around secondary area when they slipped falling forward. They stated that when they were falling hard hat fell off and strike their head. They had a open wound on their head and was taking to the urgent clinic for further evaluation.

Excavator was working to bring down boulders from top deck of mud-cell #1 of all the fresh shot and boulders. EE came down to the bottom and tried pulling boulders. The boulder was in back and sloughed off and rolled down into cab. breaking the glass.

Employee was on a 988 loader cleaning in Secondary, not being aware of the surroundings. Employee hit the Transformer Electrical box with the bucket. No injuries/illness.

The plant loader loading one of our contractors haul truck had a big rock on the edge of the bucket. When EE went to dump the bucket into the bed of the haul truck, the big rock fell and landed on hydraulic cap and two steps on the walkway damaging the steps and hydraulic cap.

EE hot the drill on the left side corner. At the time of accident there were no injuries and EE did not claim any illness. Three days after the accident EE called about their back hurting. EE then went to the doctors and claimed workers compensation and they declined it.
