Coming out of excavator, slipped and hurt his knee. He did not report it till 12:45 PM. Another EE stated he stumbled on cable and fell and twisted his knee.

There are four 1/2 inch bolts that secure the coupling on the hose. Employee took one of the bolts off and as he was taking off the second bolt the line coupling separated and caused hydraulic fluid to spray into his left eye causing medical treatment at once.
