EE WAS DRILLING WITH AN AIR TRACK DRILL, DURING THE PROCESS OF OPERATION HE WAS CHANGING DRILL STEEL AND SOMEHOW HIS RIGHT HAND GOT CAUGHT BETWEEN A SPROCKET & ROLLER CHAIN. EE CANNOT RECALL HOW OR WHY HIS HAND GOT NEAR THE SPROCKET. EE'S RIGHT HAND WAS SO SEVERLY INJURED THAT THE DR.'S HAD NO CHOICE BUT TO AMPUTATE ABOVE THE RIGHT WRIST.

I NAME WAS TIGHTENING A CAPSCREW ON A HOIST OF A DUMP TRUCK THE CAP BROKE LOOSE FROM THE HOIST SPRAYING HIM WITH HYD OIL THIS CAUSED HIM TO FALL AND HIT THE FUEL TANK OF THE TRUCK AND THE GROU ND WE FOUND OUT THAT THE TREADS ON THE HOIST GOT STRIPPED AND THATS WHAT CAUSED ACCIDENT
