Toro Rock Products · Fall from scaffolds, walkways, platforms

Employee was on cat walk helping pull 5 runs of 535kcmil cables, when the cables started to get stuck they stepped out of hand rail to push cables off hand rail when they did, they did not let go of the 535kcmil. The weight of the cables pulled them off of the deck on to the ground, Resulting in the employee fracturing forearm.

Contract EE was helping replace a belt. Slack gave way knocked him off balance he tried to get his balance his knee gave out. He said that his knee has given out a few times before prior to employment, from past injury. He went to medical facility and was put in a brace and crutches. Conditions contributing are: sudden movement, soft ground, history of knees going out, weight.

While moving some material from behind the Primary Crusher he hurt his right arm. He said it bothered him that evening and he thought it would feel better the next day. He notified me the following day that it still bothered him.
