EMPLOYEE WAS TRANSPORTING A LOAD OF ROCK TO THE CRUSHER IN A PIT TRUCK. AS HE WAS DRIVING DOWN THE ROAD TO THE CRUSHER HE APPARENTLY LOST CONTROL, VEERED TO THE SIDE OF THE ROAD, STRIKING 2 B ERMS APPROX 80' APART. THE EMPLOYEE WAS EJECTED FROM TEH CAB OF THE TRUCK AT POINT OF FINAL IMPACT.

EE was holding the top of a pipe to connect to a water pump, when the pipe slipped, causing EE's left index finger to get caught between the pipe and a beam. EE's left index finger tip and part of the finger nail were severed off.

Employee was cutting a piece of a conveyor belt on section 2. He was holding the loose piece with his right hand, while putting pressure to cut with his left hand, the utility knife slipped cutting thru his right glove and right thumb. He got stitches to his right thumb. Utility knife use has been changed for scissors to eliminate the hazard in this activity.
