March 15, 2023 PA · Coal maintenance man, mechanic, repair/serviceman, boilermaker, fueler, tire tech, field service tech SLIP OR FALL OF PERSON

Mechanic was hoisting antifreeze drum on to a rack while standing on an A-frame ladder. A gust of wind caused the drum to sway, and the mechanic lost balance and fell from the ladder a distance of 2 to 3 feet to the ground. The mechanic completed the shift but later went to the hospital where mechanic was diagnosed with left side 9 & 10 rib fractures.

Keystone Anthracite Company Incorporated · Accident type, without injuries

The haul truck suffered an engine failure which caused an outer piece of the lower engine block to blow apart. The hole opening in the block caused motor oil to spray around the outside of the engine and the hot turbo causing an ignition. The fire caused extensive damage to the operators compartment and engine.

Employees were changing hoist rope on Manitowoc 4600 dragline when wire rope got caught on berm. Wire rope struck employee when it became dislodged.
