TRP-Los Banos Plant Metal/Non-Metal

Quality control technician was adjusting the forks on a Gradall forklift & placed right hand between the back of the fork and the back of the attachment, resulting in a laceration to the right ring finger and small finger.

While running to assist customer, EE felt a pain in EE's right knee. EE was treated at clinic and was prescribed work restrictions.

On 11/7/19 the employee received stitches to close a laceration. While hosing the catwalk near the feeder, the feeder became clogged, and a rock fell out striking the employee on EE's hand resulting in a laceration to the right index finger.

Employee requested medical treatment on 2/21/18 and was prescribed work restrictions by the occupational physician. The employee alleged that ee stepped on a rock, felt two pops in L-knee and felt pain. This is an alleged, non-witnessed questionable event.

On 8/16/17, the employee received medical treatment for a right wrist fracture, was placed on restricted duty which was accommodated and subsequently lost time on 8/28/17. The employee's pant was caught on a fence post that was leaning against a truck while walking backward carrying a fence with a co-worker. This caused EE to fall backward; EE used arm to break the fall.

On 1-25-16 the employee received medical treatment for a L-wrist fracture and lost time from work. The employee, while descending a loader, missed the last step and fell backwards onto the ground.

Early morning hours, to past of speed for corner. Top heavy load on trailer. Trailer turned on side taking truck with it.

The employee went to the company clinic on 9/5/08 and was placed on restricted duty; R-knuckle required sutures. The employee cut R-hand (ring finger knuckle) on shop truck tail gate access cover plate while removing spare water truck tire.

Employee went to see company doctor on 4/3/05 and was placed on restricted work. While returning tools to the shop, employee stepped onto unlevel ground and twisted L-ankle.

EMPLOYEE WAS ATTEMPTING TO REMOVE FEEDBOX/SPLASH PAN TO 6'X16" VIBRATING SCREEN WITH THE HELP OF TWO COME-ALONGS. THE SCREEN FEEDBOX SWUNG DOWN AND STRUCK HIS LEFT THIGH CAUSING A CONTUSION AF TER BEING UNBOLTED FROM SCREEN. EMPLOYEE WENT TO SEE COMPANY DOCTOR ON 8/27/03 AND WAS PLACED ON RESTRICTED DUTY.

EE WAS LIFTING A GARBAGE CAN LOADED WITH METAL &TRASH FROM A DUMPSTER & STRAINED HIS LOWER BACK.HE WENT TO COMPANY DOCTOR ON 4/16 & WAS PLACED ON RESTRICTED DUTY 1 DAY & TEMP. TOTAL DIS. 1 DAY

EMPLOYEE WENT TO SEE THE DOCTOR ON 3/7/01 WHO PRESCRIBED PRESCRIPTION MEDICATION AND WAS RETURNED TO FULL DUTY. WHEN CROSSING UNDERNEATH CONVEYOR INSPECTION BELT TRAVEL, DIRT FELL FROM CONVEYO R BEHIND EMPLOYEE'S SAFETY GLASSES INTO RIGHT EYE.

EE WENT TO SEE DR. ON 7-24 WHO APPLIED STITCHES.EE JUST FINISHED A REPAIR, HIS RIGHT ARM WAS EXTENDED UP AND WHEN HE LOWERED IT HE CUT IT ON THE GUARD.

USING A HAMMER TO HIT OFF EDGES FROM A PIECE OF METAL FRAGMENT HIT LEFT EYE BELOW THE GOGGLES.

WORKING ON SCRUBBER WITH HAMMER & HIT OWN INDEX FINGER.

INJURED WAS PICKING UP A PIECE OF IRON OFF OF CONVEYOR BELT AND CUT ARM ON STOP CORD SUPPORT BRACKET.

WAS REPLACING SCREEN ON WASH PLANT. SCREEN SLIPPED BACK AND HIT HIS KNEE CAP.

WAS UNBOLTING 8" PIPE TO REMOVE IT. PIPE FELL AND LANDED ON FOOT.

EE WAS INSIDE SCRUBBER AND COWORKER WAS ON THE OUTSIDE TIGHTENING A BOLT. THE BOLT, UNDER PESSURE, BROKE AND SHOT UP, HITT EE IN THE EYE.

CLEANIGN AROUND THE WASH PLANT. SLIPPED ON WET SPOT CAUSING HIM TO DO THE SPLITS. THAT CAUSED A GROIN PULL ON THE INSIDE OF HIS LEG.

CLEANING GRIZZLEY ON CRUSHING PLANT. POSSIBLE STRAINED LEFT SHOULDER.

PULLING A CONVEYOR BELT AND TWISTED HIS BACK.

DRIVING EUCILUD PUT TRUCK. OPENED SIDE WINDOW DUST BLEW INTO EYE CAUSING IRRATATION WENT TO LOCAL EMERGENCY ROOM.
