Quality control technician was adjusting the forks on a Gradall forklift & placed right hand between the back of the fork and the back of the attachment, resulting in a laceration to the right ring finger and small finger.

While running to assist customer, EE felt a pain in EE's right knee. EE was treated at clinic and was prescribed work restrictions.

On 11/7/19 the employee received stitches to close a laceration. While hosing the catwalk near the feeder, the feeder became clogged, and a rock fell out striking the employee on EE's hand resulting in a laceration to the right index finger.

Employee requested medical treatment on 2/21/18 and was prescribed work restrictions by the occupational physician. The employee alleged that ee stepped on a rock, felt two pops in L-knee and felt pain. This is an alleged, non-witnessed questionable event.

On 8/16/17, the employee received medical treatment for a right wrist fracture, was placed on restricted duty which was accommodated and subsequently lost time on 8/28/17. The employee's pant was caught on a fence post that was leaning against a truck while walking backward carrying a fence with a co-worker. This caused EE to fall backward; EE used arm to break the fall.
