THERE WAS A WOODEN FRAME BUILT OVER THE LADDER AT THE END OF THE PLATFORM SUCH THAT THE TOP HORIZONTAL MEMBER OF THE FRAME HAD TO BE PASSED UDNER TO GET ONTO THE LADDER. WHEN THE EE GRABBED TH E MEMBER TO SWING UNDERNEATH, IT BROKE LOOSE CAUSING THE FALL.
EE WAS ROCKBOLTING THE MAIN DIFT WHERE WE HAD REMOVE SOME TIMBER WE HAD INSTALLED 5 MATS WITH BOLTS AND WAS STARTING ON THE SIXTH MAT HE DRILLED THE HOLE FOR THE BOLT AND HAD STARTED TO INSERT THE BOLT WHEN A SLAB CAME OUT OF THE BACK HITTING THE JACKLEG KNOCKING HIM DOWN THEN BROKE INTO SEVERAL PIECE THE PIECE HITTING HIM.

EMPLOYEE WAS UNLOADING A 55 GAL DRUM OF DIESEL FUEL OUT OF THE BUCKET OF A 1 YD UG LOADER, WHEN THE OPERATOR MOVED THE LOADER BACK HITTING THE FIRE DOOR. OEPRATOR THEN MOVED FOREWARD HITTING THE FIRE DOOR. OPERATOR THEN MOVED FORWARD HITTING THE FUEL BARREL WITH THE LOADER BUCKET, PUSHING IT INTO THE EMPLOYEE, CAUSING HIKM TO PULL THE MUSCLES IN HIS BACK.
WORKER WIN ASSESTING AN ANGLE IRON STORAGE RACK WHEN SOCIETY OF RATCKET SLIPPED OFF BOLT HEAD MOMENTENT ALLOWED HAND TO SMASH INTO ANKLE IRON RIGHT INDEX FINGER WAS CUT REQUIRING 5 STITCHES.
