EE WAS ROCKBOLTING THE MAIN DIFT WHERE WE HAD REMOVE SOME TIMBER WE HAD INSTALLED 5 MATS WITH BOLTS AND WAS STARTING ON THE SIXTH MAT HE DRILLED THE HOLE FOR THE BOLT AND HAD STARTED TO INSERT THE BOLT WHEN A SLAB CAME OUT OF THE BACK HITTING THE JACKLEG KNOCKING HIM DOWN THEN BROKE INTO SEVERAL PIECE THE PIECE HITTING HIM.

OPERATOR THEN MOVED FORWARD HITTING THE FUEL BARREL WITH THE LOADER BUCKET, PUSHING IT INTO THE EMPLOYEE, CAUSING HIKM TO PULL THE MUSCLES IN HIS BACK.
