Lifting of fabricated steel bar resulting in a lower back muscle pull.

Employee claims that the seat in the 980 cat loader caused back problems. That the seat did not have lumbar support. Employee was observed on 5/6/2015 not being able to walk with out pain, EE was ask if hurt at work, EE replied no that back has been bothering EE. A work comp claim was not filed until 8/27/2015. Employee never filed report of injury at work.

Employee lifted pit truck hood and felt back pain in his back. He continued working for approximately one month at this time he went to the doctor on 07/16/2013. The doctor put employee on restricted work duty at this time. Because of the restricted work duty, we have been unable to accommodate this employee. He is still currently off of work.

Employee was working on a muffler with a hole in it. He had his finger near the hole when another employee kicked the muffler. Hole flexed and cut his left pinky fingertip with fingernail also.

THE EMPLOYEE DIDN'T NOTICE ANYTHING AT THAT TIME, BUT OVER THE WEEKEND BEGAN TO NOTICE SOME S ORENESS IN HIS RIGHT ARM AND SHOULDER AREA. NOTIFIED EMPLOYER ON 1-31-95 THAT HIS SHOULDER AND ARM WERE SORE. ON 2-1-95 HE DETERMINED THAT HE WOULD GO TO THE DOCTOR.

THIS OCCURED ON 4-7-94, AT WHICH TIME EE STATED THAT HIS WRIST SORE. THEN ON 4-11-94, EMP INFORMED HIS SUPERVISOR THAT ON 4-7-94 HE FELL AND HURT HIS WRIST AND IT WAS VERY SORE.
