Employee was climbing stairs on right side of pit truck and said EE had wet shoes and slipped on ladder step. When EE fell EE put out left arm and is believed EE has a fracture in the forearm.

Hearing Loss. Annual hearing test showed change in hearing. Retest showed same. All testing on Equipment shows in compliance. Employee is trained on use of hearing protection, and cause if not used. Not sure this is work related.

Mechanic was welding a patch on a discharge chute. A piece of hot slag fell on mechanic's coveralls, burned through his jeans, and started a small fire. The mechanic's right calf was burned.
