MULTI-PLATE WAS BEING REMOVED FROM THE PORTAAL AT THE MAIN INTAKE. A PIECE OF MULTI-PLATE WAS BEING PULLED AWAY FROM FROZEN GROUND. A CLEVIS THAT WAS BEING USED SPREAD APART & THE LOAD FELL IT STRUCK KOWLOK IN THE HEAD.
