EE WAS HOLDING PRESSURE WITH A SLEDGE HAMMER AGAINST A CONVEYOR CHAIN ON THE MINER FOR A FELLOW EE WHILE THE CONVEYOR CHAIN WAS BEING SPLICED. FELLOW EE HIT THE CHAIN WITH A BALL PIN HAMMER AND IT GLANCED OFF AND HIT EE'S RING FINGER ON HIS RT. HAND.
A ROOF FALL OCCURRED IN INTERSECTION 1 BREAK OUTBY SPAD #893 MEASURING 30'LONG, 20'WIDE, 5-6'HIGH. 42" RESIN BOLTS FELL ABOVE ANCHORAGE. NO EQUIPMENT WAS INVOLVED. THE VENTILATION WAS NOT DIST URBED. NO INJURY OCCURRED. THE FALL WILL NOT BE CLEANED UP.

EMPLOYEE WAS DISLODGING MUD AND ICE FROM MOVING BELT CONVEYER WITH 5' PIECE OF REBAR WHEN THE REBAR BECAME CAUGHT BETWEEN THE IDLE ROLLER AND MOVING BELT CONVEYER DRAGGING INJURED THROUGH MOVI NG BELT CONVEYER AND IDLE ROLLER.
