The employee was using his pocket knife to cut a hole in his belt to shorten it. The knife folded causing a laceration to his right index finger.

Employee was adjusting a fork on the skid steer, his left hand was pushing the fork in to adjust while holding the fork with his right hand.His right hand slipped off the end of the fork, the fork fell quickly crushing the tip of the left long finger.

Employee was loading scrap steel into loader bucket when he threw a large piece into bucket causing it to bounce out and strike him on the left side of his head. His hearing aid broke inside his ear canal. Employee went to the ER to remove the foreign objects from his ear canal.
