Employee's right thumb was caught between a post hole driver and a ground rod while driving rod into ground. Employee received a laceration which required medical attention and stitches.

Miner was using a pipe wrench to loosen spray nozzles on wet screen. When the threads loosened, the miner's right hand came into contact with the spray bar above causing a laceration requiring stitches to close. Miner was released to full duty.

Miner states their knee buckled at ~5:30am while dismounting CAT 775 haul truck at end of night shift on 06/11/2017. Miner did not report the incident and went home. Miner called their supervisor at ~4:30pm on 06/11/2017 to report the right knee was swollen. Miner went to local ER diagnosed with knee strain. Miner was released back to work with restriction 06/12/2017.

On 03/02/2014 miner reported that his eye had become irritated at the end of his shift while picking up for the day. High Easterly winds prevailed most of the day on 03/01/2014. Miner was seen at local ER. No object found in his eye. Given medication for scratches and irritation and released to work.

EE was installing belt wiper on BC02 to divert stone. EE used his personal pocket knife to "mark" the belting in order to punch holes in the belting for mounting. While EE was "marking" the belt, his knife closed, causing a laceration to his right ring finger. EE was seen at local hospital, required 6 stitches to close wound, and released without restrictions.

Miner was grinding in front of welders while re-lining a pan feeder. He had his back to the welders. Welding flash came from behind his face shield and reflected off of the inside causing burns to the miner's eyes.

EE was helping replace a bearing on a speed sensor on a conveyor that was at ground level. He was walking up a small (approx. 4 foot) mound of material. His right foot slipped and he fell to same level. He struck his right shoulder on the ground, causing a right side shoulder sprain.

EMPLOYEE WAS USING A 24" CRESCENT WRENCH TO LOOSEN A RUSTED TURNBUCKLE THAT WAS SECURED IN A BENCH VISE. AS HE PULLED BACK ON THE WRENCH HANDLE WITH BOTH ARMS, THE WRENCH CAME OFF THE TURNBUCKLE CAUSING THE EMPLOYEE TO LOSE HIS BALANCE AND TURN HIS LEFT ANKLE. THE LEG FRACTURED AT THE BASE OF THE LEFT FIBULA.

TRANSFORMER BANKS WERE ISOLATED CIRCUITS. UNEXPECTED SHORT IN METER CAUSED AN ELECTRICAL ARC CAUSING BURNS TO HAND.

EE WAS REPLACING CENTER PIN ON 988 CAT LOADER. USING CUTTING TORCH TO CUT CENTER PIN THAT WAS STUCK IN A BEARING. HEAT FROM TORCH CAUSED GREASE IN BEARING TO EXPAND AND IGNITE SPRAYING EE WITH BURNING GAS. EE SUFFERED BURN ON FACE FROM HOT GREASE.
